健脑益智汤结合认知训练治疗急性脑梗死后认知障碍的疗效观察  

Observation on Curative Effect of Jiannao Yizhi Decoction Combined with Cognitive Training in the Treatment of Cognitive Impairment after Acute Cerebral Infarction

摘　　要：目的:研究健脑益智汤结合认知训练治疗急性脑梗死后认知障碍的疗效和作用机制。方法:选择急性脑梗死后认知障碍患者95例,随机分汤药+认知训练组、认知训练组、对照组3组,分别在治疗前和治疗4周采用简易精神状况量表(MMSE)、事件相关电位(P300)评定认知功能及事件相关电位的改变。结果:汤药+认知训练组总有效率87.5%,认知训练组为84.4%,对照组为58.1%,汤药+认知训练组、认知训练组与对照组疗效差异有统计学意义(P<0.05),汤药+认知训练组治疗前后P300潜伏期、波幅差异均有统计学意义(P<0.05);认知训练组治疗前后P300潜伏期差异有统计学意义(P<0.05)。Pearson相关分析显示,P300潜伏期改变与MMSE评分负相关(r=-0.893,P=0.000<0.001),波幅改变与MMSE评分无相关关系(r=-0.232,P=0.316>0.1)。结论:健脑益智汤结合认知训练治疗急性脑梗死后认知障碍是一种有效方法。Objective ：To study the curative effect and action mechanism by Jiannao Yizhi Decoction combined with cognitive training treatment on the acute cerebral infraction patients with cognitive impairment. Methods ：95 acute cerebral infraction patients with cognitive disorder were randomly divided into the Jiannao Yizhi Decoction plus cognitive training group, cognitive training group and control group. All cases were evaluated by the mini - mental state examination and e- vent - related potential （ P300 ） characteristic before and after 4 weeks treatment respectively. Results ： The total effective rate of traditional Jiannao Yizhi Decoction plus cognitive training group was 87.5%, of cognitive training group was 84. 4% , of the control group was 58.1% , the difference was significant（ P 〈 0.05 ）. The latency and amplitude in the Jiannao Yizhi Decoction plus cognitive training group compared before and after treatment were significantly different （P 〈 0.05 ）. The latency in the cognitive training group compared before and after treatment was significantly different（ P 〈 0.05 ）. The correlation analysis showed that latency change and MMSE score were negative related （ r = - 0. 893, P = 0. 000 〈 0. 001 ） , and there was no correlation between amplitude change with MMSE score（ r = -0.232 ,P = 0.316 〉 0.1 ）. Con- clusion：Jiannao Yizhi Decoction combined with cognitive training is effective on treating cognitive impairment after acute cerebral infarction.
