摘　　要：目的通过检测结直肠癌组织中生长激素受体(GHR)和胰岛素样生长因子-Ⅰ(IGF-Ⅰ)的表达情况,探讨GH-IGF-Ⅰ轴在结直肠癌发生发展及预后中的作用。方法应用免疫组织化学SP法检测结直肠癌组织、癌旁组织和正常组织中GHR和IGF-Ⅰ的表达情况,采用信访或电话访问形式收集患者生存资料。一致性检验用Kappa分析,分类资料的差异性比较用χ2检验,应用Kaplan-Meier法计算术后生存率,Log-rank检验和Cox回归分别用于预后单因素和多因素分析。结果①GHR、IGF-Ⅰ在癌组织中表达的阳性率显著高于癌旁组织和正常组织(P<0.05)。②GHR、IGF-Ⅰ在癌组织中的表达与患者性别、年龄、肿瘤直径、分化程度、Dukes分期、TNM分期、淋巴结转移及远处转移等均无明显相关。③癌组织中GHR和IGF-Ⅰ表达的相关性具有统计学意义(P<0.05)。结论 GH-IGF-Ⅰ轴在结直肠癌的发生发展及预后中发挥一定的作用,但作用不明显,尚需进一步研究。Objective To detect the expression of GHR and IGF-Ⅰ in colorectal cancer and their relationship with clinicopathological factors and prognosis,and to explore the roles of GHR and IGF-Ⅰ in the development and prognosis of colorectal cancer.Methods Immunohistochemical SP method was applied to detect the expression of GHR and IGF-Ⅰ in cancer tissues,adjacent tissues and normal tissues of 127 patients with colorectal cancer.Survival information was collected through mail or telephone interview.Kappa analysis was applied to test the consistency,categorical data were analyzed by chi-square test,and Log-rank test and Cox proportional hazard regression model was applied for univariate and multivariate analysis respectively.Results The expression of GHR and IGF-Ⅰ in cancer tissues was significantly higher than that in adjacent tissues and normal tissues（P0.05）.There was no significant association between the GHR and IGF-Ⅰ and clinicopathological factors.There was significant correlation between the expression of GHR and IGF-Ⅰ in cancer tissues（P0.05）.Conclusion GHR and IGF-Ⅰ play roles in the development and prognosis of colorectal cancer to some extent.
