血清铁蛋白对幼年特发性关节炎全身型诊断价值的研究

摘　　要：目的探讨血清铁蛋白(SF)在幼年特发性关节炎全身型(SO-JIA)诊断中的价值。方法收集医院2000～2011年住院治疗的63例SO-JIA作为观察组,同时随机收集医院同期其他疾病(幼年特发性关节炎其他型、感染性疾病、肿瘤、其他结缔组织病)104例作为对照组,30例健康体检儿童作为健康组,采用化学发光法检测SF水平并分别进行统计学分析。结果 SO-JIA患儿血清SF水平较对照组及健康组显著升高(P<0.01);SO-JIA组患儿的SF水平均高于正常值,SO-JIA组升高5倍以上达43例,占68.3%;升高10倍以上15例,占23.8%;根据ROC曲线,其最靠近左上方的点为SF=1269.50 mg/L,敏感性为95.8%,特异性为92.4%。结论 SF是诊断SO-JIA非常有价值的实验室指标,SF=1269.50 mg/L可能是一个比较好的诊断SO-JIA截断点。【Objective】 To investigate the diagnostic value of serum ferritin levels in children with system onset Juvenile idiopathic arthritis（SO-JIA） at the time of Clinical significance.【Methods】 The serum ferritin levels were detected by Chemiluminescence in 63 children with SO-JIA and 104 other patients（other Juvenile idiopathic arthritis,infectious diseases,cancer and other connective tissue diseases）,at the same time the serum ferritin levels of 30 healthy children were determined.【Results】 The serum ferritin level was signally higher in children with SO-JIA than that in other two groups（P 0.01）;the levels of serum ferritin in SO-JIA group were all higher than normal and the levels of serum ferritin in 68.3% of SO-JIA group were more than five-fold elevation,and the levels of serum ferritin in 23.8% of SO-JIA group were more than ten-fold elevation.When the cut-off levels of serllm ferritin was 1269.50 mg/L,the sensitivity for the diagnosis was 95.8% respectively,the specificity were 92.4% respectively.【Conclusions】 Serum forritin is a valuable laboratory parameters for the diagnosis of SO-JIA and 1269.50 mg/L may be a good cut-off level.
