肝动脉栓塞化疗联合立体定向适形放疗对原发性肝癌患者的疗效观察

摘　　要：目的观察肝动脉栓塞化疗联合立体定向适形放疗对原发性肝癌患者的疗效。方法收集本院收治的未接受手术治疗的原发性肝癌患者66例,按治疗方式分为肝动脉栓塞化疗联合立体定向适形放疗的观察组43例,和单纯接受肝动脉栓塞化疗的对照组23例。观察两组患者的近期疗效、远期疗效,治疗前后甲胎蛋白(AFP)的变化及不良反应的发生等。结果观察组患者的近期治疗有效率为72.1%,显著优于对照组(43.5%);观察组患者总生存时间和一年生存率均显著优于对照组;2组患者治疗前AFP值无显著差异,治疗后观察组患者AFP显著低于对照组;2组不良反应的发生无统计学差异。结论肝动脉栓塞化疗联合立体定向适形放疗可显著提高患者的治疗效果。Objective To observe the therapeutic efficacy of transcatheter hepatic arterial chemoembolization combined with stereotaxis conformal therapy in patients with primary hepatic carcinoma. Methods Sixty-six patients with primary hepatic carcinoma who didn＇t receive opera- tion in our hospital were selected. Forty-three patients who received transcatheter hepatic arterial chemoembolization combined with stereotaxis conformal therapy were divided into observation group. The rest 23 patients who received transcatheter hepatic arterial chemoembolization were di- vided into control group. The short-term curative effects, long-term curative effects, the changes of AFP and the occurrence of adverse events in both groups were observed. Results The short-term curative effect in the observation group was 72.1%, which was significantly higher than that in the control group. The total survival duration and one-year survival rate in the observation group were all significantly higher than those in the control group. There were no significant differences be- tween the two groups in the level of AFP before therapy. The level of AFP after therapy in the ob- servation group was significantly lower than that in the control group. The occurrences of adverse events in these two groups showed no significant differences. Conclusion Transcatheter hepatic ar- terial chemoembolization combined with stereotaxis conformal therapy could improve the curative ef- fects in patients with primary hepatic carcinoma.
