摘　　要：目的探讨正常胃组织、良性病变组织和胃癌组织中CD44v6和ICAM-1的表达水平,并观察它们与临床病理特征尤其是淋巴结转移的关系。方法应用免疫组化SP法,对78例胃癌组织、30例胃良性病变组织、30例正常胃组织中CD44v6和ICAM-1的表达进行检测,并探讨其与临床病理特征尤其是淋巴结转移的关系进行检测。结果胃癌组织中CD44v6和ICAM-1表达阳性率显著高于胃良性病变及正常胃组织(P<0.05)。胃癌组织中CD44v6阳性率在低分化组明显高于高、中分化组(P<0.05),CD44v6和ICAM-1在有淋巴结转移组明显高于无淋巴结转移组(P<0.05),在T3、T4组明显高于T1、T2组(P<0.05),在Ⅲ、Ⅳ期组明显高于Ⅰ、Ⅱ期组(P<0.05)。联合检测CD44v6和ICAM-1在胃癌组织中的表达可明显提高检测胃癌转移的敏感性和特异性(P<0.05)。结论 CD44v6与胃癌的分化程度、浸润深度、淋巴结转移和TNM分期密切相关,ICAM-1与胃癌的浸润深度、淋巴结转移和TNM分期密切相关。综合分析上述两指标,能明显提高检测胃癌转移的敏感性和特异性,更准确的判断胃癌的转移情况。Objective To investigate the expression of CD44v6 and cellular adhesion molecule （ICAM-1） in normal gastric tissue, benign lesion tissue and gastric carcinoma tissue, and to explore their relation with clinical pathological characteristics as well as lymph node metastasis in gastric carcinoma. Methods The expression levels of CD44v6 and ICAM-1 were detected in 78 cases of gastric carcinoma tissue,30 cases of gastric benign lesion tissue and 30 cases of normal gastric tissue by immunohistochemistry assay （SP） and to explore the correlation between the expressions of CD44v6 and ICAM-1 and pathological characteristics as well as lymph node metastasis in gastric carcinoma. Results The positive expression rates of CD44v6 and ICAM-1 in gastric carcinoma tissue were significantly higher than those in gastric benign lesion tissue and normal gastric tissue （ P 〈 0.05 ）. The positive rate expression of CDdZtv6 in poor differentiation group was significantly higher than that in well differentiation group and moderation differentiation group （ P 〈 0.05 ）. The expression levels of CD44v6 and ICAM-lin lymph node metastasis group were obviously higher than those of non lymph node metastasis group （ P 〈 0.05 ）, which in T3 group and T4 group were significantly higher than those in T1 group and T2 group （ P 〈 0.05） ,and the expression levels of CD44v6 and ICAM-1 in Ill, IV phase groups were significantly higher than those in I , II phase groups （ P 〈 0.05 ）. The combined detection of the levels of CIM4v6 with ICAM-1 could enhance diagnostic sensitivity and specificity for lymph node metastasis in gastric carcinoma （ P 〈 0.05 ）. Conclusion The expression of CD44v6 is closely related with the differentiation degree, infiltration depth, lymphatic node metastasis and TNM staging of gastric carcinoma. The combined detection of the levels of CD44v6 with ICAM-1 can enhance diagnostic sensitivity and specificity for lymph node metastasis in gastric carcinoma.
