摘 要:目的检测大肠癌组织中Ang-2、Tie-2和VEGFR-2的表达水平,探讨3者在大肠癌发生、复发转移、预后及血管生成中的调节作用。方法采用免疫组SP方法,检测118例大肠癌组织中、40例癌旁组织及40例肠道良性疾病组织中Ang-2、Tie-2及VEGFR-2的表达及CD34标记的微血管(MVD)进行检测,分析它们与大肠癌血管生成的关系。结果大肠癌组织中Ang-2、Tie-2及VEGFR-2在的阳性表达率分别为74.58%、69.49%和61.02%明显高于癌旁组织中的25.00%、17.50%和17.50%及大肠良性病变组织中的35.00%、32.50%和32.50%;(P值均<0.05);大肠癌组织中2个或3个因子共同表达率为61.02%,明显高于单个因子阳性表达率(15.25%);癌组织中MVD表达量为(31.43±10.50),明显高于癌旁组织中的(10.61±3.76)和大肠良性病变组织中的(16.89±3.83)(P<0.05)。三者表达与CEA和MVD均呈正相关(P<0.05)。Ang-2的表达与CA19-9呈正相关(P<0.05),Tie-2和VEGFR-2的表达则与CA19-9无关(P>0.05);根治术后5年内复发转移者和未复发转移者Ang-2、Tie-2和VEGFR-2的表达差异均有统计学意义(P<0.05),阴性表达者5年生存率明显高于阳性表达者(P<0.05)。结论 Ang-2、Tie-2和VEGFR-2在大肠癌血管生成过程中发挥重要作用,其高表达与大肠癌的发生、复发转移和预后有关。Objective To study the expression of angiotensin-2 (Ang-2), Tie-2 and vascular endothelial growth factor receptor-2 (VEGFR-2) in colorectal cancer and analyze their relationship with the occurrence, recurrence, metastasis, angiogenesis and prognosis of colorectal cancer. Methods Immunohistochemistry with SP method was used to detect the expressions of Ang-2, Tie-2 and VEGFR-2 in 118 colorectal cancer, 40 adjacent normal tissue and 40 benign colorectal lesion specimens. Results The positivity rates of Ang-2, Tie-2 and VEGFR-2 in colorectal cancer tissue were 74.58% , 69.49%, and 61.02% , respectively, significantly higher than those in the adjacent normal tissues (25.00%, 17.50%, and 17.50%, P〈0.05) and benign colorectal lesion tissues (35.00%, 32.50%, and 32.50%, P〈0.05). The rates of two or three coexpressions were significantly higher than that of a single expression in the cancer tissues (61.02% vs 15.25%). The microvascular density (MVD) of colorectal cancer tissues was 31.43±10.50, significantly higher than that of the adjacent normal tissues (10.61±3.76) and benign colorectal lesions (16.89±3.83) (P〈0.05). The expressions of Ang-2, Tie-2, and VEGFR-2 were positively correlated with carcinoembryonic antigen (CEA) and MVD (P〈0.05). The expression of Ang-2, but not Tie-2 and VEGFR-2, was positively correlated with CA199. Ang-2, Tie-2, and VEGFR-2 expressions showed significant differences between cases with tumor recurrence/metastasis and those withouts 5 years after radical mastectomy, and were all positively correlated with the 5-year survival rates (P〈0.05). Conclusion Ang-2, Tie-2 and VEGFR-2 are involved in the development, invasion, metastasis, and prognosis of colorectal cancer, and play important roles in the angiogenesis of the tumors.
