摘 要:目的 探讨胰腺内分泌肿瘤中外分泌细胞成分的存在及其来源。方法 应用免疫组织化学技术 (S P法 )对 5 7例胰腺内分泌肿瘤 (非功能性胰腺内分泌肿瘤 2 3例 ,胰岛素瘤 34例 )进行上皮膜抗原 (EMA)的染色 ,对肿瘤中外分泌细胞成分的存在和分布进行观察。结果 2 1例 (36 .8% )胰腺内分泌肿瘤中具有EMA阳性染色。阳性细胞多构成导管或腺泡样组织学结构。部分阳性细胞具有与正常外分泌细胞不一致的组织学形态和EMA染色特征。结论 胰腺内分泌肿瘤中具有一些胰腺外分泌组织学结构和细胞成分 ,支持胰腺肿瘤共同起源学说即多能干细胞理论。Objective\ To study the components of exocrine cells in pancreatic endocrine tumor (PET) and discuss their biological origin. Methods\ 57 PET patients were immunohistochemically investigated for expressioin of epithelial membrane antigen (EMA) by using S P method and the histological distribution of positive staining cells (representing exocrine cells) were observed. Results\ EMA immuno reactive cells could be found in 21 ( 36.8% ) PETs, most of them were located in tubular or acinar structures. Some of the positive staining cells possessed histological and immunohistochemical features nonconsistent with normal pancreatic exocrine cells. Conclusion\ Exocrine structures and cell components exist in pancreatic endocrine tumors, supporting the theory that pancreatic tumors may have the same derivation from multiple stem cells.
