GP方案治疗晚期肺鳞癌与肺腺癌的临床观察

摘　　要：目的观察GP方案对60例晚期肺鳞癌与肺腺癌治疗的疗效差别,为晚期肺癌患者个体化治疗的筛选提供依据。方法肺鳞癌组30例与肺腺癌组30例均接受吉西他滨(1000mg/m2)加顺铂(75mg/m2)化疗,每3周重复,每2个周期评价疗效,最多6周期。结果两组患者共60例均可评价,鳞癌组与腺癌组有效率分别为26.7%和30%(P>0.05),两组疾病控制率分别为80%和83.3%(P>0.05)。鳞癌组中位疾病进展时间5.0个月,中位生存期12.4个月,1年生存率50.0%,2年生存率3.3%。腺癌组中位疾病进展时间5.8个月,中位生存期15.4个月,1年生存率63.3%,2年生存率10.0%。两组差异均无统计学意义(P>0.05)。主要的不良反应为骨髓抑制和消化道反应,均可耐受。结论吉西他滨联合顺铂对晚期肺鳞癌与肺腺癌的疾病控制率相近,腺癌表现出比鳞癌更好的生存优势。是晚期非小细胞肺癌一线化疗的优选方案之一。Objective To investigate the difference in the therapy effects of Gemcitabine combined with Cisplatin on 60 patiens with advanced pulmonary squamous carcinoma and adenocarcinoma,and provide a good choice for individuate treatment.Methods Thirty patiens with advanced pulmonary squamous carcinoma and thirty patiens with advanced pulmonary adenocarcinoma received intravenously GEM 1000mg/m2 on d1,d8,puls Cisplatin 75mg/m2,every three weeks as a treatment cycle,and the effects were evaluated by CT every two cycles.Results Sixty cases were appraised in the two groups.The response rate RR（CR＋PR） of the advanced pulmonary squamous carcinoma and adenocarcinoma was 26.7% and 30%,the disease control rate DCR（CR＋PR＋SD） of the two groups was 80% and 83.3%.TTP of the two groups was 5.0 months and 5.8 months.Median survival time was 12.4 months and 15.4 months.The 1-year survival rate was 50.0% and 2-year survival rate was 3.0% in the pulmonary squamous carcinoma group.The 1-year survival rate was 63.3% and 2-year survival rate was 10.0% in the adenocarcinoma group.The main side effects were myelosuppression and alimentary toxicities.All that can be tolerated.Conclusion The control of advanced pulmonary squamous carcinoma and adenocarcinoma treated by Gemcitabine combined with Cisplatin is similar but the existence adenocarcinoma is higher than that of pulmonary squamous carcinoma.
