亚低温治疗大面积脑梗死疗效的系统评价

摘　　要：目的系统评价亚低温治疗大面积脑梗死(MCI)的疗效。方法通过检索近10年来Cochrane Library、Pubmed、Web of Science、Embase、Medline、中国生物医学文献数据库(CBM)、中国学术期刊全文数据库(CNKI)、万方数据库(CECDB)、维普中文科技期刊全文数据库(CQVIP),对纳入研究的方法学进行评价,对文献质量进行严格评价和资料提取,对符合质量标准的随机临床试验(RCT)用Review manager 5.0软件进行Meta分析。结果 6个RCT共251名MCI患者纳入研究,其中治疗组(在常规治疗基础上使用亚低温治疗)124例,对照组(常规治疗)127例。Meta分析结果表明治疗组患者住院时间、神经功能缺损程度评分(CSS)、临床失智评分(CDR)明显低于对照组,具有显著性差异(P均<0.05)。结论相比常规治疗,亚低温治疗可显著提升MCI患者的疗效,值得在临床诊疗中广为普及。Objective It is to systematic evaluate the curative effect of mild hypothermia therapy in massive cerebral infarction.Methods The data of Cochrane Library,Pubmed,Web of Science,Embase,Medline,CBM,CNKI,CECDB and CQVIP nearly ten years was retrieved.The internalized studies＇ methodology was evaluated;their quality was inspected by strictness evaluation and data extraction.The randomized-controlled trials（RCT） consistent with quality standards were used for Meta-analysis by Review manager 5.0 Software.Results Total 6 RCTs of 251 patients included in the study,there are 124 cases in treatment group（mild hypothermia therapy and conventional treatment）,and 127 cases in control group（conventional therapy）.Meta-analysis results showed that the duration of hospitalization in patients,CSS,CDR in treatment group was significantly lower than that in control group,there was significant difference（P0.05）.Conclusion Compared to conventional treatment,the treatment of mild hypothermia can markedly increase the curative effect in MCI patients;it is worth popularity in the clinical diagnosis and treatment.
