摘　　要：目的:探讨Livin和半胱天冬蛋白酶-3(Caspase-3)蛋白在胃癌组织中的表达及临床意义。方法:(1)用免疫组化PV-9000二步法检测76例胃癌组织和30例癌旁正常组织中Livin和Caspase-3蛋白表达情况。(2)用原位末端标记(TUNEL)法检测胃癌组织中细胞凋亡情况。结果:(1)Livin和Caspase-3分别在胃癌组织和癌旁正常组织中的阳性表达率具有统计学意义(P<0.05)。(2)Livin阳性表达率与组织分化程度、浸润深度、淋巴结转移相关(P<0.05);Caspase-3的阳性表达率与组织分化程度、淋巴结转移相关(P<0.05)。(3)Livin阳性组平均凋亡指数(AI)低于阴性组(P<0.05);Caspase-3阳性组AI高于阴性组(P<0.05)。(4)有随访结果的56例中,5年总生存率为46.8%。单因素和多因素分析显示,Livin和淋巴结转移是影响胃癌患者预后的独立风险因素(P<0.05)。结论:(1)Livin和Caspase-3在胃癌组织中的异常表达,导致细胞凋亡不足,可能是胃癌形成的机制之一。(2)Livin和淋巴结转移是影响胃癌患者预后的独立风险因素。Objective To investigate the expressions and the clinical significance of Livin and Caspase-3 in gastric cancer. Methods The expressions of Livin and Caspase-3 in 76 cases of gastric cancer tissues and 30 cases of adjacent normal gastric mucosa were detected by immunohistochemieal staining （PV-9000 2-step plus）. The apoptosis of 76 cases of gastric cancer tissues was detected by TDT-mediated DNA in situ end labeling （TUNEL） method. Results The expressions of Livin and Caspase-3 in gastric cancer tissues were significantly different from those in the adjacent normalgastric mucosa （P 〈 0.05）. The expression of Livin was closely correlated with the differentiation, depth, and lymph node metastasis of gastric cancer; and that of Caspase-3 was correlated with the differentiation and lymph node metastasis （P〈 0.05）. The average apoptosis index in Livin positive group was significantly lower than that in Livin negative group （P 〈 0.05）, and that in caspase-3 positive group was significantly higher than that in caspase-3 negative group （P 〈 0.05 ）. The total five-year survival rate was 46.8% in 56 patients. Univariant analysis and multivariate analysis showed that lymph node metastasis and Livin were independent prognostic factors for patients with gastric cancer （P 〈 0.05）. Conclusions The abnormal expressions of Livin and Caspase-3 in gastric cancer tissues may play a role in the occurrence and development of gastric cancer. Lymph node metastasis and Livin were the independent prognostic factors for patients with gastric cancer.
