腹腔镜对68例女性非对称性内生殖器发育异常的应用价值

摘　　要：目的:探讨腹腔镜对女性不对称性内生殖器官发育异常的诊断及治疗价值。方法:对1998-2008年我院收治的腹腔镜诊治的68例不对称性生殖器官发育异常的患者资料进行回顾性分析。结果:68例患者中残角子宫49例,单角子宫7例,残角子宫合并单角子宫7例,阴道斜隔综合征5例。合并子宫腺肌症7例,卵巢巧克力囊肿6例,腹膜型子宫内膜异位症5例,盆腔粘连16例,输卵管伞闭锁7例。进行残角子宫切除25例;卵巢巧克力囊肿剔出9例;盆腔粘连松解13例,输卵管整形6例;5例阴道斜隔综合征患者中3例进行腹腔镜下阴道斜隔切开手术,2例加用宫腔镜下辅助切开。结论:腹腔镜下直视诊断和治疗不对称性生殖道发育异常有独特的诊断价值,有助于制定治疗方案,为女性生殖器官发育异常的快速、准确诊断开辟了新的途径,并能提供及时有效的治疗。Objective To explore the diagnostic and therapeutic value of laparoscopy in asymmetric reproductive organ malformation in female. Method Data of patients with reproductive organ malformation, who were hospitalized from 1998 to 2008 were analyzed retrospectively. Result Among 68 patients, there were 49 cases with rudimentary horn of uterus, 7 with uterus unicornis, 7 with rudimentary horn of uterus plus uterus unicornis, and 5 cases with oblique vaginal septum syndrome. 7 cases were complicated with adenomyosis, 6 cases with chocolate cyst of ovary, 5 with abdominal endometriosis, 16 with pelvic adhesion, and 7 cases with atresia of fimbria of oviduct. 25 cases received resection of rudimentary horn of uterus, 9 cases received rejection of chocolate cyst of ovary, 13 eases received lysis of pelvic adhesion, 6 cases underwent reshaping of oviduct, 3 of 5 cases with oblique vaginal septum syndrome received incision of oblique vaginal septum under laparoscopy, the other 2 cases were assisted by hysteroscopy. Conclusion There was significant value of laparoscopy in diagnosing and treating asymmetric reproductive organ malformation.
