代币激励法在社区精神分裂症患者药物自我处置技能训练的应用

摘　　要：目的探讨代币激励法对社区精神分裂症患者药物自我处置技能训练的效果。方法将80例社区精神分裂症患者随机分为两组,对照组40例,实验组40例,对照组在药物自我处置技能训练中给予恰当的鼓励、表扬。实验组授课中予以口头表扬及社会性支持语言,授课结束时予以代币激励法以示鼓励。在实验结束时,同步对两组患者进行上课率、作业完成率、知识掌握率进行测评。随访实验结束3个月、6个月的药物依从性和药物自我管理能力,并进行比较。结果实验组患者作业完成率、知识掌握率、上课率均高于对照组(分别为:χ2=8.66,6.77,15.36;P<0.01)。实验结束后3个月、6个月的药物依从性和药物自我管理能力明显高于对照组(分别为:χ2=7.17,6.67,7.82,7.92;P<0.01)。结论代币激励法能提高社区精神分裂症患者药物自我处置技能训练的作业完成率、上课率、知识掌握率,能提高患者的药物依从性和药物自我管理能力。Objective To explore the effect of token incentive act on the medication management capacity train of community schizophrenics.Methods A total of 80 community schizophrenics were randomly divided into two groups,control group of 40 patients,40 patients in experimental group.The control group was encouraged and praised at the right time in the drug disposal capacity of training.The experimental group was praised orally and afforded social support language in the medium of instruction,tokens would be gave to them to show encouragement after the instruction.At the end of instruction,we evaluated attendance rate,job completion rates and knowledge of master rate of two groups simultaneously.Then,we compared drug compliance and ability of self-encouragement within three months and six months after the follow-up experiment.Results Job completion rates,knowledge of master rate and attendance rate of patients in experimental group were higher than those in control group（Respectively for χ2=8.66,6.77,15.36;P0.01）.Drug compliance and medication management capacity of patients in experimental group were significantly higher than those in control group（Respectively for χ2=7.17,6.67,7.82,7.92;P0.01）.Conclusion Token Incentive Act can improve the job completion rate,attendance rate,knowledge of master rate,and increase drug compliance and drug self-management capacity of community schizophrenics.
