神经内镜下经单鼻孔-蝶窦切除大型垂体腺瘤38例临床研究

摘　　要：目的探讨神经内镜下经单鼻孔-蝶窦切除大型垂体腺瘤的疗效和安全性。方法采用随机数字表法将本院收治的76例巨大型垂体腺瘤患者随机分为观察组和对照组,每组各38例。观察组采用神经内镜下经单鼻孔-蝶窦切除手术治疗,对照组采用显微镜手术治疗,比较两组患者的手术时间、术中出血量、手术并发症发生率和手术治疗效果。结果观察组手术时间为(91.40±20.47)min、术中出血量为(45.60±11.73)mL、住院时间(11.40±3.69)d、手术并发症发生率为23.68%,与对照组比较差异具有统计学意义(均P<0.05)。观察组肿瘤全切除26例,次全切除7例,大部分切除5例;对照组全切除24例,次全切除5例,大部分切除9例,两组间比较差异具有统计学意义(u=7.142,P<0.05)。结论采用神经内镜下经单鼻孔-蝶窦切除大型垂体腺瘤具有手术并发症发生率低、患者术后恢复快等优点,值得临床研究推广应用。Objective To discuss the efficacy and safety of endoscopic single nostrils the nerve-sphenoid sinus resection for large pituitary adenomas. Methods Seventy-six cases of great type pituitary adenomas were randomly divided into ob- servation group and control group by random number table in our hospital, each group of 38 cases. The neural endoscopic single nostrils-sphenoid sinus surgery treatment were used in observation group, the microscope surgery were used in con- trol group, the operation time, peri-operative bleeding, surgical complications and surgical treatment effect of two groups patients were compared. Results The operation time of observation group was （91.40±20.47） rain, peri-operative bleeding of observation group was （45.60±11.73） mL, the length of time of observation group was （11.40±3.69） d, surgical complica- tion rate of observation group was 23.68%, observation group and observation group were compared, statistically significant difference （all P 〈 0.05）. The subtotal resection of observation group were 26 cases, sub-total were 7 cases, most resection were 5 patients, and the total excision of control group were 24 cases, sub-total were 5 patients, and most of the resection were 9 cases, differences between two groups were comparared, statistically significant difference （u = 7.142, P 〈 0.05）. Conclusion The neural endoscopic transsphenoidal approach by the single nostrils-the questions fur large adenoma resec- tion have low surgery complications, the postoperative recovery etc, it is worth for clinical research application.
