摘 要:目的探讨Hedgehog信号通路相关蛋白在乳腺原发癌与相应淋巴结转移癌中的表达及意义。方法采用免疫组化PV-6000法检测57例乳腺浸润性导管癌的原发灶和相应淋巴结转移灶,以及35例癌旁正常乳腺上皮组织中Shh、Ptch、Smo和Gli-1蛋白的表达。结果在正常乳腺导管上皮中Shh、Ptch、Smo和Gli-1蛋白表达呈阴性或弱阳性,而在乳腺原发癌灶中Shh、Ptch、Smo和Gli-1蛋白表达的阳性率分别为91.2%(52/57)、86%(49/57)、91.2%(52/57)和71.9%(41/57);在其相应淋巴结转移灶中的阳性率分别为82.5%(47/52)、91.2%(52/57)、75.4%(43/57)和22.8%(13/57)。Shh和Ptch蛋白表达在乳腺癌原发灶和其淋巴结转移灶中的差异均无统计学意义(P均>0.05),而Smo和Gli-1蛋白表达在乳腺癌原发灶和其相应淋巴结转移灶中的差异有统计学意义(P<0.05和P<0.01),且Smo与Gli-1蛋白表达在乳腺癌原发灶和转移灶中均呈正相关(P均<0.01)。结论 Shh和Ptch蛋白在乳腺癌原发灶和淋巴结转移灶中均呈高表达,且表达一致;而Smo和Gli-1蛋白在乳腺癌原发灶中的表达明显高于淋巴结转移灶,尤其是Gli-1蛋白在原发灶的高表达可能在乳腺癌的侵袭和淋巴道转移过程中起到调控的作用。Purpose To investigate the associated protein expression and significance of Hedgehog signaling pathway in primary breast cancer and matched lymph node metastases. Methods The protein expressions of Shh, Ptch, Smo and Gli-1 were detected by immu- nohistochemistry in a series of 57 human invasive breast cancer and matched lymph node metastases, and 35 cases of normal breast ductal epithelial tissues. Results The expression of Shh, Ptch, Smo and Gli-1 in normal breast ductal epithelial tissues were com- pletely absent or rare. The positive rates of Shh, Ptch, Smo and Gli-1 in primary breast cancer were 91.2% (52/57), 86% (49/ 57), 91.2% (52/57) and 71.9% (41/57), the positive rates in the matched lymph node metastases were 82. 5% (47/52), 91.2% (52/57), 75.4% (43/57) and 22. 8% (3/57). Shh and Ptch expression had no differences in primary breast cancer and matched lymph node metastases (P 〉 0. 05 ). While Smo and Gli-1 expression had differences in primary breast cancer and matched lymph node metastases (P 〈 0.05 and P 〈 0.01 ) and Smo expression was significantly correlated with Gli-1 both primary breast cancer and matched lymph node metastases (P 〈 0.01 ). Conclusion The expression of Shh and Ptch protein in primary breast cancer and lymph node metastases are in high level and show the consistency of expression. Smo and Gli-1 expression in primary cancer are signifi- cantly higher than those in the lymph node metastases. Especially, the high expression of Gli-1 protein in primary breast cancer may play an important role in the invasion and lymph node metastases of the human invasive breast cancer.
