CT引导下经胸膜腔外路径行纵隔肿瘤穿刺活检术的临床研究

摘　　要：目的探讨CT引导下经胸膜腔外路径行纵隔肿瘤穿刺活检术的应用价值。方法选择46例可疑纵隔肿瘤患者为研究对象,行CT引导下经胸膜腔外路径纵隔肿瘤穿刺活检术。与手术后病理学检查结果进行对比研究,采用灵敏度、特异度、气胸率、咯血率来评价CT引导下经胸膜腔外路径行纵隔肿瘤穿刺活检术的临床应用价值。结果 CT引导下经胸膜腔外路径行纵隔肿瘤穿刺活检术和手术后病理检查的诊断效果无明显统计学意义(P>0.05),CT引导下经胸膜腔外路径行纵隔肿瘤穿刺活检术进行肿瘤诊断的灵敏度为80.00%,诊断的特异度为78.95%,漏诊率为20.00%,误诊率为21.05%。结论 CT引导下经胸膜腔外路径行纵隔肿瘤穿刺活检术是一种新的纵隔肿瘤穿刺活检路径,能显著提高纵隔肿瘤穿刺活检术的安全性和准确性。Objective To explore the value of CT guided biopsy outside of the pleural cavity in mediastinal tumor.Methods 46 patients suspected of mediastinum tumor were selected,all patients underwent by CT guided biopsy outside of the pleural cavity,compared with pathological examination after surgery,the sensitivity,specificity,rate of pneumothorax and rate of hemoptysis were evaluated.Results There was no statistical significant difference in diagnostic effect between biopsy outside of the pleural cavity and pathological examination after surgery（P0.05）,the sensitivity of cancer diagnosis was 80.00%,the specific diagnosis of 78.95%,the missed diagnosis was 20.00%,the misdiagnosis rate was 21.05% in CT guided biopsy outside of the pleural cavity group.Conclusion CT guided biopsy outside of the pleural cavity is an effective and safe method in diagnosis of the mediastinum tumor.
