微种植体或Nance弓支抗矫治上颌前突的临床对比研究

摘　　要：目的:对比研究以微种植体作支抗及以Nance弓作支抗矫治上颌前突的临床效果。方法:选取80例安氏Ⅱ1错牙合畸形病例,随机分为两组,均采用滑动直丝弓矫治技术,实验组40例使用微种植体支抗;对照组40例采用Nance弓支抗。通过对两组治疗前后的头颅侧位X线片进行头影测量分析,评价两组病例在上颌前突矫治中软硬组织的变化。结果:实验组上前牙后移4.93 mm,磨牙近中移动0.48 mm,上前牙后移的平均速度1.38 mm/月,矫治疗程平均16.6个月,对照组上前牙后移3.69 mm,磨牙近中移动1.02 mm,上前牙后移速度平均1.05 mm/月,矫治疗程平均20.5个月。结论:微种植体和Nance弓支抗均可取得显著临床疗效,但微种植体支抗可使上前牙更多内收,且可有效地缩短矫治时间。Objective： To give an account of the clinical effect of the anchorage micro-implants used in the treatment of maxillary protrusion cases. Method： 80 patients who are Angles class Ⅱ, division 1 malocclusion are chosen and classified into two groups by random and the two groups are both used the straight wire appliance sliding mechanics. The experimental group chooses the anchorage micro-implants and the matched group chooses the NANCE bow as the anchorage. We can assess the change of the hard and soft tissue following the treatment of maxillary protrusion cases using the anchorage micro- implants and calculate the speed of the front teeth＇ backward movement, by analyzing the cephalometrics of the two groups before and after the treatment. Result： In the experimental group, the front teeth＇ backward movement is 4.93 mm, the molars＇ mesial movement is 0.48 mm, the average speed of the front teeth＇ backward movement is 1.38 mm/month,the average treatment procedure is 16.6 months;In the matched group,the front teeth＇ backward movement is 3.69 mm,the molars＇ mesial movement is 1.02 mm, the average speed of the front teeth＇ backward movement is 1.05 mm / month, the average treatment procedure is 20.5 months. Conclusion： Although the anchorage micro-implants and the NANCE bow both can reach the pronounced effect,the anchorage micro-implants can make the front teeth adduct more and shorten the time of treatment effectively.
