摘 要:目的探讨晚期肺癌患者(Ⅲ~Ⅳ期)胸水及外周血中T细胞受体Vβ亚家族的表达,比较二者的异同。方法收集2010年7月至2012年5月共89例临床诊断为肺癌晚期的患者胸水及外周血标本,采用流式细胞仪方法测定其T细胞受体Vβ亚家族的表达情况,统计分析胸水及外周血中Vβ亚家族表达的异同。结果 89例晚期肺癌外周血和胸水均主要高表达Vβ3和Vβ9,分别为Vβ3(16/89)、Vβ9(18/89)和Vβ3(20/89)、Vβ9(15/89);患者胸水与外周血Vβ亚家族存在差异,部分差异有统计学意义(P<0.05),部分差异无统计学意义(P>0.05),其中胸水中Vβ3的高表达相对于患者外周血和健康人差异均有统计学意义(P<0.05)。结论晚期肺癌患者胸水及外周血中Vβ亚家族的表达不全相同;晚期肺癌胸水中的高表达Vβ亚家族少于外周血,其中Vβ3在胸水中可能有特异性。此研究为晚期肺癌的生物治疗及T细胞过继免疫疗法在临床中的应用奠定理论基础。Objective To investigate and compare the expression of T cells receptor (TCR) Vβ subfamilies in peripheral blood and pleural effusion in patients with advanced lung cancer (Ⅲ~Ⅳ stage). Methods Specimens from peripheral blood and pleural effusion of 89 patients diagnosed as advanced lung cancer from July 2010 to May 2012 were collected. The expression of TCR Vβ subfamilies were detected in the specimens using the flow cytometry (FCM). Statistical analysis was performed. Results The peripheral blood and pleural effusion showed high expression of Vβ3 and Vβ9, specifically Vβ3 (16/89) and Vβ9 (18/89), Vβ3 (20/89) and Vβ9 (15/89), respectively. Difference was found in the expression of TCR Vβ subfamilies between peripheral blood and pleural effusion, of which some were statistically significant (P0.05) and some were not (P0.05). The high expression of Vβ3 in pleural effusion showed statistically significant difference with that in peripheral blood and healthy individuals (P0.05). Conclusion The expression of TCR Vβ subfamilies are not identical in peripheral blood and pleural effusion in patients with advanced lung cancer, The TCR Vβ subfamilies highly expressed in pleural effusion are less than those in peripheral blood, and Vβ3 might be specific in pleural effusion. This study provides theoretical foundation for the application of Biotherapy and T cells Adoptive Immunotherapy in the treatment of advanced lung cancer.
