功能性消化不良患者食管运动功能的变化

摘　　要：探讨功能性消化不良（ＦＤ）患者食管运动功能变化。方法：采用 ＰＣ ｐｏｌｙｇｒａｆ ＨＲ台式高分辨上消化道气液压毛细管连续灌注测压系统对４０例ＦＤ患者和２０例正常人进行食管测压。结果：ＦＤ组食管下段蠕动压低于上段蠕动压的发生率显著高于对照组；ＦＤ组食管下段平均蠕动压显著低于对照组；ＦＤ组出现异常蠕动波２１例，表现为类型不同，次数不等的双峰波、三峰波、逆行收缩波、自发性收缩波和非同步收缩波等，以双峰波最多见。９例同时伴有食管下段低蠕动压；ＦＤ组食管体部运动异常检出率为７５％。结论：ＦＤ患者存在食管运动功能异常，主要表现在食管体部运动不协调，其次为食管下段运动减弱。Aim: To explore the esophageal motility changes in patients with FD. Methods: Esophageal pressure was measured in 40 patients with FD and 20 healthy individuals as contrast by means of catheter perfusion technigue (pc polygraf). Results: The occurrence rate of lower esophageal peristalsis pressure of FD group was lower than that of the upper of the same group, but it was significantly higher than that in the contrast group. The average peristalsis amplitude of contraction of lower segment of esophageal body in patients with FD was significantly lower than that in the control group. The ahnormal peristalsis waves in FD made up 21 cases, which was displayed as double - peak , three - peak, retropulsion and spontaneous contration waves, etc, in different types and times, among which double - peak were found mostly having lower esophageal amplitude in 9 cases. The incidence rate of esophageal motility abnormalities was 75% in FD group. Conclusion: The esophageal motility disfunction could occur in patients with FD, which is characteristed mostly by disorder of esophageal body movement, and secondly by decrease of lower esophageal movement.
