早期子宫颈鳞癌中podoplanin标记的淋巴管密度及其临床意义

摘　　要：目的探讨早期子宫颈癌组织中肾小球上皮细胞整合膜蛋白(podoplanin)标记的淋巴管密度(LVD)及其临床意义。方法免疫组化SP法检测38例Ⅰb1期子宫颈鳞状细胞癌和10例非癌性宫颈组织中podoplanin标记的LVD,分析其与临床病理分级的关系。结果癌组织中心及边缘组织淋巴管密度与正常子宫颈组织相比均明显增加(P<0.05,P<0.05);癌灶边缘组织淋巴管密度为12.10±2.83条/例,明显高于癌灶中心部位淋巴管密度7.71±1.12条/例(P<0.05)。有淋巴结转移者癌灶边缘组织LVD 14.67±3.72条/例,无淋巴结转移者10.77±2.65条/例,差异有统计学意义(P<0.01);组织学分级G1、G2和G3组癌灶边缘LVD分别为8.29±1.76条/例,11.31±3.63条/例,13.24±0.79条/例。G2组LVD明显高于G1组而低于G3组(P<0.05,P<0.05)。结论早期宫颈癌组织中LVD的增加有临床意义,其与肿瘤淋巴转移,细胞分化等临床病理因素相关,宫颈癌组织中淋巴管的生成可能在其淋巴转移过程中发挥重要作用。Objective To explore the expression and clinical significance of lymphatic vessels density（LVD） stained by podoplanin in early stage of cervical carcinoma.Methods Immunohistochemical staining by podoplanin was used to detect LVD in 38 cases of early stage of carcinoma of uterine cervix and 12 cases of normal uterine cervix,and the correlation between LVD and tumor clinicopathological features was analyzed.Results The peritumoral LVD（PLVD）and the intratumoral LVD（ILVD）were significantly higher than that of normal uterine cervix（P0.05）.The peritumoral LVD was 12.10 ± 2.83,which was much more than that in the intratumoral carcinoma（7.71 ± 1.12）（P0.05）.The PLVD with lymphatic metastasis was14.67 ±3.72,which was more than that without lymphatic metastasis（10.77±2.65）（P0.01）.The PLVD of histological grade G1,G2 and G3 were 8.29±1.76,11.31±3.63 and 13.24±0.79,respectively.The PLVD of G2 was more than that of G1 but lower than that of G3（P0.05）.Conclusion The increase of LVD in early stage carcinoma of uterine cervix had clinical significance.It was relevant with the clinicopathological factors,such as lymphatic metastasis and cell differentiation.The generation of lymphatic vessels may play an important role in the lymphatic metastasis of cervical carcinoma.
