胸腔镜辅助下与开放式食管癌根治术的综合临床效果比较

摘　　要：目的比较胸腔镜辅助下食管癌根治术与开放式食管癌根治术的临床疗效。方法选择2008年1月-2011年12月医院收治的60例食管癌患者为研究对象,将患者随机分为观察组和对照组组各30例。观察组采用胸腔镜辅助下食管癌根治术治疗,对照组采用常规左胸入路开放式食管癌根治术治疗,比较两组手术时间、住院时间、术中出血量、术后并发症发生率及术后生存率。结果观察组手术时间明显长于对照组,术中出血量、住院时间明显少于对照组,术后并发症发生率明显低于对照组,差异均有统计学意义(P<0.05)。经过1年的随访,两组术后1年生存率比较差异无统计学意义(P>0.05)。结论胸腔镜辅助下食管癌根治术治疗食管癌虽然手术时间延长,但是出血量少、并发症少、住院时间短等,安全、可行。Objective To compare and analyze the clinical efficacy of thoracoscopy laparoscopy and open esophageal cancer surgery.Methods 60 cases of esophageal cancer patients admitted to hospital from January 2008o December 2011 were chosen as the research object.According to the random number table,patients were randomly divided into the observation group and the control group,and 30 cases were in each group.The observation group was taken with thoracoscopy and laparoscopy radical mastectomy,and the control group was treated with three incision of the neck chest and abdomen path radical mastectomy.The operative time,hospitalization time,blood loss,postoperative complication rate and postoperative survival rate were compared.Results The operative time of the observation group was significantly longer than that of control group,but intraoperative blood loss and hospital stay was significantly less than that of the control group,the incidence of postoperative complications was significantly lower than that of the control group,the differences were statistically significant（P 0.05）.After 1 year of follow-up,the difference of postoperative 1-year survival rate was not statistically significant（P 0.05）.Conclusion The operation time for thoracoscopy and laparoscopy surgical treatment of esophageal cancer is long,but it has the advantage of less bleeding,fewer complications and shorter hospital stay.It is safe and feasible.
