恶性肿瘤合并2型糖尿病患者化疗后血清肿瘤标志物及血糖的变化

摘　　要：目的研究恶性肿瘤合并2型糖尿病患者接受化疗后血清肿瘤标志物及血糖的变化特点，探讨化疗对恶性肿瘤合并2型糖尿病患者肿瘤标志物水平及血糖值的影响。方法34例患者，分为2组，A组为恶性肿瘤合并2型糖尿病患者，B组为恶性肿瘤未并发糖尿病者，化疗前后按时检测2组患者的血糖值及各肿瘤标志物水平，计算2组患者化疗后各肿瘤标志物的下降率。分析患者化疗后血糖和肿瘤标志物的变化规律，以及肿瘤标志物水平的变化是否与合并2型糖尿病相关。结果2组患者化疗后空腹血糖（FBG）均明显升高（P〈0．05），且B组3例出现继发性糖耐量异常（IGT）。2组患者化疗后血清肿瘤标志物水平均不同程度下降（P〈0．05），B组较A组降低明显但下降率差异无统计学意义（P〉0．05）。结论恶性肿瘤患者化疗后血糖均明显升高，血清肿瘤标志物水平均降低；合并2型糖尿病对肿瘤标志物水平的变化无明显影响。Objective To investigate the change and characteristic of chemotherapy on tumor marker and blood glu- cose in patients with malignant tumor with concomitant type 2 diabetes mellitus and try to find the influential factors of chemotherapy. Methods 34 inpatients divided into two groups. Group A were afflicted with malignant tumor with type 2 diabetes mellitus and group B were that of malignant tumor without concomitant diabetes mellitus. Their blood glucose and tumor marker were detected before and after chemotherapy. Clinical data were collected simultaneously, to analyze the change regularity of tumor marker and fasting blood sugar （FBG） after chemotherapy, and whether the change of tumor marker level were related with type 2 diabetes mellitus. Results Group A showed higher FBG than that before chemo- therapy （P 〈 0.05）, such data also had the same obvious change in patients of group B （ P 〈 0.05 ）, and 3 cases of group B showed abnormal glucose tolerance. Following chemotherapy, tumor marker level in both groups decreased to a certain degree （ P 〈 0.05 ）. The data after chemotherapy in group B was lower than that in group A, but with no statisti- cal difference （ P 〉 0.05 ）. Conclusions Patients with malignant tumor, their FBG and tumor marker all reach a higher level than that before chemotherapy, whether with concomitant type 2 diabetes mellitus does not influence the change of tumor marker significantly.
