双极电凝钳与电凝钩配合施行腹腔镜结直肠癌根治术

摘　　要：目的:探讨双极电凝钳与电凝钩配合施行腹腔镜结直肠癌根治术的可行性、安全性和优越性。方法:回顾性分析接受该手术方式的25例结直肠癌(A组)和25例开腹结直肠癌根治术的临床资料(B组),对比两组患者术中出血量、手术时间、术中副损伤、术后首次肛门排气时间、术后腹痛、腹胀程度、术后并发症及远期疗效。结果:50例手术皆顺利完成,全部康复出院。A组手术出血量少于B组、术后首次肛门排气时间短于B组、A组平均手术时间长于B组,有显著统计学意义(P<0.01),两组肿瘤直径、淋巴结清扫数差异均无统计学意义(P>0.05)。A组术后腹痛轻于B组(P<0.05),两组腹胀程度差异无统计学意义(P>0.05)。两组各有1例吻合口瘘和1例切口感染,均经治疗好转。随访1～24个月无复发。结论:双极电凝钳与电凝钩配合施行腹腔镜结直肠癌根治术是一种安全可行、更微创美观、更利于保肛且降低手术费用的一种手术方式。Objective：To investigate the the feasibility,safety and superiority bipolar for laparoscopic bipolar combined with electric coagulation forceps and electrocautery in colorectal cancer resection.Methods：Retrospective analysis of the clinical data acceptance of the operation mode of 25 patients with colorectal cancer（A group） and 25 cases of open colorectal cancer resection of colorectal cancer（B group）,compared to two groups of patients with bleeding in the operation,operation time,intraoperative side injury,after first anal exhaust time,postoperative abdominal pain,abdominal distention,postoperative complications and long-term outcome.Results：50 patients were successfully completed,all recovered and were discharged.Surgical blood loss in group A was less in group B,postoperative first flatus was less in group A,operative was longer in group A than group B,there were statistically significant（P0.01）,two groups of tumor size,lymph node dissection,the number was no significant difference（P0.05）.A group of postoperative abdominal pain was lighter in group A than group B（P0.05）,the two sets of abdominal distension of the difference was not statistically significant（P0.05）.Two groups each with 1 case of anastomotic fistula and 1 case with infection of incisional wound,were recovered after treatment.Follow-up for 1 ~ 24 months without recurrence.Conclusion：Laparoscopic bipolar combined with electric coagulation forceps and electrocautery in colorectal cancer resection of colorectal carcinoma is a safe and feasible,minimally invasive aesthetics,and more conducive to the anal sphincter preservation,and a surgical approach to reduce the cost of surgery.
