摘 要:目的探讨青春前期孤独症患儿与正常儿童性激素水平差异,以及其可能的临床意义。方法应用直接化学发光法检测44例孤独症患儿[男35例/女9例,平均年龄(47.5±18.9)月]和44例正常对照儿童[男35例/女9例,平均年龄(45.0±18.7)月]的血清卵泡刺激素(Follicle—stimulatinghormone,FSH)、黄体生成素(Luteotropichormone,LH)、睾酮(Testosterone,T)、雌二醇(Estradiol,E2)、孕酮(Proges—terone,P)、硫酸脱氢表雄酮(Dehydroepiandrosteronesulfate,DHEA-S)和雄烯二酮(Androstenedione,A)水平,并进行统计分析。结果①孤独症组儿童血清孕酮水平(〈0.48,2.08)nmol/L低于正常对照组儿童(〈0.48,6.95)nmol/L,差异具有统计学意义(Z=3.564,P〈0.01),而FSH、LH、T、E2、DHEA—S、A激素水平两组之间差异均无统计学意义(z分别为0.831,1.167,1.091,0.906,0.830,0.826,均P〉0.05);②〈48月和≥48月孤独症组患儿血清孕酮水平无统计学差异(Z=0.150,P=0.881);男性和女性孤独症组患儿血清孕酮水平亦差异无统计学意义(Z=1.972,P=0.249)。结论孤独症患儿孕酮水平降低,且与性别、年龄无关,提示其水平变化与孤独症的发生密切相关。Objective To investigate the changes of the serum sex steroids levels in prepuberal children with autism, and analyze the possible clinical implications. Methods The serum sex steroids, including follicle- stimulating hormone ( FSH), luteotropic hormone ( LH), testosterone (T) , estradiol ( E2 ), progesterone ( P), de- hydroepiandrosterone sulfate(DHEA-S) and androstenedione were measured in 44 autistic children(35 boys and 9 girls, 47.5 ±18.9 months) and 44 normal children(35 boys and 9 girls ,45.0 ±8.7 months), using chemilumi- nescence immunoassay. Results (1)The progesterone levels in autism group were significantly lower than control group( ( 〈 0.48, 2.08) nmol/L vs ( 〈 0.48, 6.95) nmol/L, Z = - 3. 564, P 〈 0.01 ), but no statistical differ- ences were observed in other sex steroids levels(P〉 0.05 ). (2)The progesterone levels in 〈 48 months autism group showed no significant differences from t〉48 months autism group ( Z= 0. 150, P〉 0.05 ). There were no statistical differences in progesterone level between the boys and girls of the autism group( Z = 1. 972, P 〉 0.05 ). Conclu- sion The progesterone levels are lower in autistic children, and they are not associated with the children's age or gender. This study indicates that the progesterone levels are closely associated with autism.
