摘  要:目的探讨轻度认知损害(MCI)、轻度及中度阿尔茨海默病(AD)患者左右侧别海马头体尾各区磁共振T2信号强度的改变。方法收集MCI、轻度及中度AD患者和老年健康志愿者各15例,所有受试者均行FSE-,12序列,并在工作站手工勾绘海马头体尾各区感兴趣区,测量其磁共振,12信号强度。并分析各部磁共振他信号强度与简易智力状态检查(MMSE)评分的相关性。结果MCI患者的左侧海马头部(569.80±15.74)、轻度AD双侧海马头部(左:573.75±16.16;右:570.21±15.43)及中度AD双侧海马头部(左:592.31±15.77;右:582.35±16.80)、左侧海马尾部(578.36±16.08)磁共振他信号强度较对照组均差异有统计学意义(P〈0.05)。同时轻度AD患者的左侧海马尾部的信号存在增高趋势。MMSE评分与MCI、轻、中度AD组的双侧海马部分亚区呈负相关。结论MCI、轻中度AD患者双侧海马部分亚区的磁共振r12信号强度存在异常,而对左侧海马头部的早期定量测量,将有助于对AD的早期诊断。Objective To investigate the change of the hippocampal MRI T2 signal intensity in mild cog- nitive impairment(MCl) and mild and moderate Alzheimer's disease,and to give imageological help to the early di- agnosis and treatment of Alzheimer's disease(AD). Methods 15 patients with MCI,15 patients with mild AD,15 patients with moderate AD and 15 age- and gender- matched healthy people were scanned by MRI using FSE-T2 sequence, and the margin of hippocampal subfields was outlined manually for each side to measure the T2 signal in- tensity. The relationship between hippocampal T2 signal intensity and the scores of MMSE was analyzed. Result- s There was significant difference of the T2 signal intensity in the left hippoearnpal head of MCI, the bilateral hip- poeampal head of mild AD and the bilateral hippocampal head , the left hippocampal tail of moderate AD (P 〈 0.05 ) ,while the numerical value of the left head of mild AD was bigger than the control group ( but P 〉 0.05 ). There was inverse correlation between some hippocampal subfields T2 signal intensity and the scores of MMSE. Conclusion There is abnormality of some hippocampal subfields T2 signal intensity, and the quantitative meas- urement early of the left hippocampal head is useful in making clinical diagnosis of AD in its early stage.
