全瓷冠联合牙槽突磨削治疗上前牙前突伴骨性前突

摘　　要：目的观察全瓷冠联合牙槽突磨削治疗上前牙前突伴轻中度上颌骨骨性前突的临床治疗效果。方法选取我科门诊收治的56例上前牙前突伴轻中度上颌骨骨性前突的病例,共412颗牙,对牙槽突进行定量磨削,切除部分牙龈,作全瓷冠内收上前牙。术后1、3、6个月和1年随访。结果全瓷冠的临床效果评价:边缘完整性、解剖外形、表面光滑度、颜色均为A。牙龈临床评价:第1个月都是A,3个月有2个B,6个月有5个B和1个C,1年有3个B。修复后1年患者满意度评价:"非常满意"43例,"基本满意"13例,无"不满意",满意度77%。专业医师评价优良率:纠正上前牙前突98.5%,减轻深覆颌深覆盖84.5%,与邻牙匹配度97.5%,整体轮廓96.6%。结论采用全瓷冠联合牙槽突磨削是治疗上前牙前突伴轻中度上颌骨骨性前突的有效方法,其远期效果有待进一步观察。Objective To observe the clinical effects of all - ceramic crown combined with grinding of dental process on the protrusion of upper anterior teeth accompanied with light or moderate bony protrusion of maxillary bone. Methods 56 cases （412 teeth） with the protrusion of upper anterior teeth accompanied with light or moderate bony protrusion of maxillary bone were selected in our out - patient clinic. Quantitative grinding of alveolar process was carried out. Part of the gingival gum was resected, and the adduction of upper anterior teeth was made within the all - ceramic crown. Follow up was made 1 month, 3 and 6 months and 1 year after the operation. Results In the clinical effect evaluation of all - ceramic crown, the scores for margin integrity, anatomy form, surface smooth degree and color were all ＂A＂. As to the clinical evaluation of gingival gum, the scores in the first month were all ＂ A＂ , and there were 2 ＂Bs＂ in month 3; there were 5 ＂Bs＂ and 1 ＂C＂ in month 6; there were 3 ＂Bs＂ in 1 year. As to the evaluation of patients＂ satisfaction 1 year after the reparation, there were ＂very satisfactory＂ in 43 cases, ＂satisfactory＂ in 13 ones, and no case of ＂dissatisfactory ＂. The satisfaction rate was 77%. As to the excellence rate evaluated by the doctors, the satisfaction rate of the upper anterior teeth protrusion rectification was 98.5% ; the relief of deep - covered jaw was 84.5% ; the matching degree with the adjacent teeth was 97.5% ; the rate of the whole outline was 96.6%. Conclusion The all - ceramic crown combined with grinding of dental process is an effective treatment on the protrusion of upper anterior teeth accompanied with light or moderate bony protrusion of maxillary bone. Its long term effects need further observation.
