高危型HPV负荷量、年龄与宫颈上皮内瘤变分级的相关性

摘　　要：【目的】探讨高危型人乳头状瘤病毒（HPV）负荷量、年龄与宫颈上皮内瘤变分级的相关性。【方法】选择本院2010年1月至2011年12月圜液基薄层细胞检测（TCT）异常和第二代杂交捕获实验（HC-Ⅱ）阳性行阴道镜下活检患者1376例，按组织学诊断分为慢性宫颈炎、宫颈上皮内瘤变（CIN）I、cINⅡ／Ⅲ三组，高危型HPV（HR—HPV）负荷量按大小分成5个等级，分析HR—HPV负荷量、年龄与宫颈上皮内瘤变分级的关系。【结果11376例患者中组织学诊断为慢性宫颈炎686例，CINI292例，CINⅡ／Ⅲ398例，HR～HPV负荷量分级与宫颈病变程度比较差异无统计学意义（P〉0．05）；30～40岁年龄组宫颈病变发病率最高，显著高于〈30年龄组及〉40岁年龄组，其差异有统计学意义（P〈0．05）。【结论】HR—HPV负荷量与宫颈病变分级无明显关系；30～40岁是宫颈病变发病的高发年龄。[Objective] To explore the correlation of high-risk（HR） human papillomavirus（HPV） load and age with the grade of cervical intraepithelial neoplasia. [Methods]Totally 1376 patients with abnormal TCT and positive HC-I by colposcopically directed biopsy in our hospital from Jan. 2010 to Dec. 2011 were cho- sen. According to histological diagnosis, all patients were divided into chronic cervicitis group, CIN I group and CIN Ⅱ / Ⅲ group. HR-HPV load was divided into 5 grades. The correlation of HR-HPV load and age with the grade of cervical intraepithelial neoplasia was analyzed. [Results]Among the 1376 patients, there were 1376 patients with chronic cervicitis, 292 patients with CIN I and 398 patients with CIN Ⅱ / Ⅲ diagnosed by histology. There was no significant difference between HR-HPV load and the degree of cervical lesion（ P〉0.05）. The incidence of cervical disease in the group with the age between 30 and 40 years old was the high-est, which was markedly higher than that in the group with the age below 30 years old and the group with the age over 40 years old, and there was significant difference（ P〈0.05）. [Conclusion]There is no obvious correlation between HR-HPV load and the grade of cervical lesion. The 30-40 years old is the age with high incidence of cervical lesion.
