摘　　要：目的　观察去甲硫氨酸全肠外营养对5FU抗癌作用的影响。方法　甲基硝基亚硝基胍(MNNG)诱发胃癌的大鼠23只,随机分为实验组[去甲硫氨酸全肠外营养(-MetTPN)加5FU化疗(n=12)]和对照组[含甲硫氨酸全肠外营养(+MetTPN)加5FU化疗(n=11)]。治疗7d后,每组随机抽取5只处死,采取标本作相应指标检测;两组其余大鼠改普通饲料,分笼饲养直至死亡,观察生存时间。结果　两组大鼠体重、肿瘤直径和肿瘤细胞DNA指数以及肿瘤细胞S期、G2M期比例在治疗后均显著下降,G0/G1期比例明显增高。-MetTPN加5FU组上述改变更明显。大鼠血清游离Met、胱氨酸(Cys)明显下降,丝氨酸(Ser)等显著上升。经-MetTPN加5FU治疗后,肿瘤组织坏死灶增多,肿瘤细胞出现网状化、脱落,细胞核固缩、碎裂,核质和核仁比例扩大;部分肝细胞和肾小管上皮细胞亦见轻度肿胀变性。荷瘤大鼠最终多死于肿瘤转移和全身衰竭,实验组大鼠平均生存期43d,对照组平均生存27.5d,两组差异有显著意义(P<0.05)。结论　去甲硫氨酸全肠外营养能增强5FU对大鼠胃癌的疗效。Objective To observe the antitumor effect of L methionine deprived total parenteral nutrition with 5?FU in gastric cancer bearing rats.Methods MNNG induced gatric cancer rats were randomly divided into experiment group [Met deprived TPN+ 5? FU ( n =12)] and control group [Met containing TPN+5?FU( n =11)].Five animals in each group were sacrificed after 7 days of treatment and the samples were taken for examination. The remaining rats in each group were changed to feed with normal diet after the treatment until death,the life span was noted.Results The body weight of rats, the diameter of tumors and the DNA index of tumor cells as well as the ratio of tumor cells's phase and G 2M phase went down in both groups. At the same time, the ratio of tumor cells G 0/G 1 phase went up. The changes were more significant in the Met deprived TPN+ 5?FU group and acompanied by remarkable decrease of serum free Met and Cys. But, some other amino acids,such as serum free Ser and Glu, were increased significantly. After 7 days treatment of methionine deprived TPN+5?FU, more necrotic foci apeared in the tumor tissue and there was shrinkage of the tumor and prolongation of the survival time. Meanwhile, there were slight degeneration in some liver and kidney cells. In closing, all the rats died of multiple organ failure caused by the metastasis of carcinoma. The average survival time was 43 days in experiment group, while it was 27.5 days in the control group( P <0.05).Conclusion Met deprived TPN can enhance the anti tumor effect of 5?FU and lead to longer life span of gastric cancer bearing rats.
