铝碳酸镁等药物对大鼠非固醇类抗炎药性胃粘膜损害的预防作用

摘　　要：目的 :探讨铝碳酸镁 (胃达喜 )对非固醇类抗炎药 (NSAIDs)性胃粘膜损害的预防作用。 方法 :以吲哚美辛 (消炎痛 )复制大鼠 NSAIDs性胃粘膜损伤模型 ,以光镜和扫描电镜观察粘膜损伤状况 ;观察预防应用铝碳酸镁、西咪替丁 (泰胃美 )和硫糖铝 (舒克菲 )后模型动物胃粘膜形态学改变 ,同时测定胃粘膜血流 (GMBF) ,6 -酮 -前列环素 F1α(6 - keto- PGF1α) ,血栓素 B2(TXB2 )水平。 结果 :吲哚美辛引起明显胃粘膜损害 ,伴 GMBF减少 ,6 - keto- PGF1α及 TXB2 水平下降 (P<0 .0 1) ,成功制备了大鼠 NSAIDs性胃粘膜损伤模型。预防应用铝碳酸镁等 3种药后吲哚美辛所致胃粘膜损害减轻 (P<0 .0 1) ,GMBF增加 (P<0 .0 1) ,6 - keto- PGF1α,TXB2 抑制减弱。 结论 :预防应用粘膜保护剂铝碳酸镁等药物可减轻 NSAIDs性胃粘膜损伤。Objective: To evaluate and compare the protective effect of talcid against NSAIDs induced gastric mucosal damage with other gastroprotective drugs such as tagamet and sucralfate. Methods: NSAIDs induced gastric mucosal damage model in rats was obtained by indomethacin. In other group, pretreatment with talcid, tagamet or sucralfate was performed 1 hour before indomethacin intubation feeding. Morphological changes of gastric mucosa were assessed by light and scanning electronic microscopy in each group. Gastric mucosa blood flow (GMBF), 6 keto PGF 1α and TXB 2 level were also measured. Results: Indomethacin caused evident gastric damage. The epithelium was in poor arrangement and more cells were destroyed severely, bleeding was common, furthermore, some of which with exposure of lamina propriar. A marked reduction of GMBF, a powerful inhibition on 6 keto PGF 1α and TXB 2 level were detected at the same time (81.2% and 83.4% respectively, P <0.01 vs control). Pretreatment with the 3 drugs could reduce NSAIDs gastric mucosa damage to certain extent. Talcid increased GMBF significantly vs indomethacin, and the lesion index was 1.04±0.35 ( P <0.01 vs indomethacin). 6 keto PGF 1α and TXB 2 level were obviously increased at the same time. Sucralfate provided the best efficacy among them. Conclusion: Pretreatment with talcid has gastroprotective effect against NSAIDs mucosal damage to a certain extent.
