卡铂复合液腹腔内化疗的抗腹腔粘连和腹膜纤维化作用

摘　　要：目的 :观察含有卡铂、透明质酸钠、人体清蛋白的卡铂复合液在胃癌、大肠癌切除术后腹腔内化疗中的抗腹腔粘连和腹膜纤维化作用。方法 :胃癌、大肠癌切除术后患者随机分为卡铂液组和卡铂复合液组 ,比较两组患者在第 2次手术时腹腔内粘连发生率和粘连程度 ,腹膜纤维化发生率和纤维化程度 ;采用寿命表分析法比较术后复发转移率。结果 :卡铂复合液腹腔内化疗可明显降低腹腔内粘连程度和粘连发生率 (P<0 .0 1) ,减少腹膜纤维化程度和纤维化发生率 (P<0 .0 1) ,癌肿切除术后采用卡铂复合液腹腔内化疗患者术后复发转移率明显低于卡铂液组。结论 :卡铂复合液可明显地抑制腹腔内化疗中常见的腹腔粘连和腹膜纤维化 ,明显提高对癌细胞杀伤作用。采用卡铂复合液腹腔内化疗是胃肠道癌肿切除后预防复发、转移的一种安全、有效方法。Objective: To evaluate the effect of the carboplatin mixture liquid, which containing carboplatin, sodium hyaluronate and albumin, on formation of intraabdominal adhesion and peritoneal fibrosis during the intraperitoneal chemotherapy in patients with gastric cancer or colorectal cancer after radical resection. Methods: Patients after radical resection of the gastric cancer or colorectal cancer were divided into the carboplatin group and carboplatin mixture group at random. The intraabdominal adhesion and peritoneal fibrosis were compared during the second operation between the 2 groups. The rates of recurrence and metastasis were evaluated by life table analysis. Results: Carboplatin mixture liquid decreased the severity, rate of the intraabdominal adhesion and the peritoneal fibrosis compared with the carboplatin liquid group. The rates of recurrence and metastasis were significantly lower in patients with the carboplatin mixture liquid than with the carboplatin liquid. Conclusion: The intraperitoneal chemotherapy with the carboplatin mixture liquid may inhibit the intraabdominal adhesion and peritoneal fibrosis, and it is much more safe and effective for prevention of recurrence and metastasis after resection. The carboplatin mixture liquid is a more suitable liquid for the intraperitoneal chemotherapy in patients with gastrointestinal cancer after radical resection.
