摘 要:目的 探讨年龄、血糖及肾功能对血清糖基化终期产物 (AGEs)水平的影响。方法 应用竞争性ELISA对正常人、慢性肾功能不全 (CRF)和 2型糖尿病 (2型DM )患者血清AGEs进行检测。结果 正常人血清AGEs水平随年龄增长而增加 ;不同年龄段肾功能损害程度相近的CRF患者之间血清AGEs水平无明显差异 ,但CRF患者血清AGEs水平均较同龄正常人群明显增加 (均为P <0 .0 0 1) ,且在CRF患者中血清AGEs水平与血清肌酐水平呈正相关 (r =0 .36 5 2 ,P <0 .0 1) ;2型DM患者血清AGEs水平较同龄正常人血清AGEs水平明显增加 (P <0 .0 0 1) ,2型DM伴肾功能损害患者血清AGEs水平较CRF患者血清AGEs水平出现增高趋势 ,但无统计学差异 (P >0 .0 5 ) ,多元逐步线性回归分析表明在 5 0岁以上的 2型DM患者中血清AGEs水平与年龄及有无眼底病变无关 ,而与血清糖化血红蛋白 (HbA1c)水平及血清肌酐水平有关 (标准化回归系数分别为 0 .6 374与0 .35 2 5 ,P分别为 0 .0 0 2 4与 0 .0 6 0 4)。结论 在正常人群年龄是影响血清AGEs水平的因素 ;在CRF患者肾功能损害程度与血清AGEs水平有关 ;在 2型DM患者长期血糖控制情况和肾功能均是影响血清AGEs水平的因素 ,其中肾功能影响可能更为重要。Objective To evaluate the effects of age, hyperglycemia and renal function on the level of serum advanced glycosylation end products (AGEs). Methods The serum AGEs were measured by competitive ELISA in 120 normal subjects, 55 patients with chronic renal failure (CRF) and 33 patients with type 2 diabetes mellitus (DM). Results (1)The serum AGEs levels of normal subjects increased with ageing. Significant differences of serum AGEs levels in normal subjects between each two adjacent groups were found (P<0.001), except for groups C and D. (2)significant increases of serum AGEs were found in CRF patients of all age groups as compared with those of normal subjects of same age group (P<0.001), but no significant difference of serum AGEs was found between two age groups (<35 year and 35~50 year) of CRF patients. A positive correlation between serum AGEs levels and serum creatinine levels was found in CRF patients (r=0.3652, P<0.01). (3)The serum AGEs level in patients with type 2 DM was significantly higher than that of normal subjects of same age (≥50year) (P<0.001), but no significant difference was found between type 2 DM patients with renal failure and CRF patients. Multiple stepwise linear regression analysis showed that serum AGEs had no relationship with age and diabetic retinopathy, but positive correlation withserumcreatinineleveland hemoglobin A 1c level in type 2 DM patients (P=0.0024, 0.0601 respectively). Conclusion (1)Age is correlated with the serum level of AGEs. (2)In CRF patients, increases of serum creatinine is associated with accumulation of serum AGEs. (3)In type 2 DM patients, both serum creatinine and hemoglobin A 1c contribute to accumulation of serum AGEs, serum creatinine being the more critical factor.
