肠上皮下肌成纤维细胞与大肠癌关系的研究

摘　　要：目的探讨肠上皮下肌成纤维细胞(ISEMF)分布,表达与大肠癌转移及预后之间的关系。方法应用免疫组化方法,采用α-平滑肌肌动蛋白(а-SMA)单克隆抗体检测ISEMF在20例正常对照者大肠黏膜及80例大肠癌中的分布和表达。结果 ISEMF主要分布于正常肠粘膜中下部,呈线状分布于腺体周围,在腺体底部与粘膜肌层形成连接;在所有大肠癌肠粘膜标本中均弥漫表达,广泛分布于间质各部分。а-SMA阳性及阳性面积之间的比例在肿瘤组织及正常肠粘膜组织中的表达分别为682154.84±510027.63和263541.57±135729.21,0.1976±0.0027和0.0799±0.0302;肿瘤组织中а-SMA表达阳性明显增高,两者有显著统计学差异(P<0.01)。结论大肠癌与正常大肠粘膜间质中ISEMF的分布存在明显差异,大肠癌肠粘膜间质中ISEMF的明显增加,提示ISEMF分布与肿瘤的组织学类型及大肠癌侵袭发展有关。Objective to investigate the relationship between the distribution and expression of ISEMFs and the metastasis and prognosis of coloree- tal cancers.Methods ISEMFs from 80 Colorectal cancers patients and 20 normal controls were detected by immunohistochemistry with smooth muscle actin moneelonal antibody（a - SMA）. Results ISEMF are mainly located in lower part of normal colonic mucosa around the gland appearing line, form- ing a connection at the bottom of the glands and mucous membrane layer; ISEMF in all expression in intestinal mucosa in colorectal cancer specimens are diffuse, broad distributing between interstitials. The ratio of A - SMA positive to positive area in tumor tissues and normal mueosa are respectively per cent 682154.84 ± 510027.63 and 263541.57 ±135729.21,0.1976 ± 0.0027 and 0.0799 ± 0. 0302; The positive of A- S M A expression significantly increased in tumor tissue with a significant statistical differences （ P 〈 0.01 ）. Condusions There are significant differences between large intestine can- cer and normal colonic mueosa of the distribution of myofrbroblasts. Myofibroblasts in the stromal area of eolorectal cancers was increased. The distribu- tion of myofibreblasts in the stromal area of colorectal cancers was related with the tumor histological type and the development of colorectal cancer invasion.
