扶正保元汤辅助晚期非小细胞肺癌化疗34例疗效观察

摘　　要：目的:观察扶正保元汤辅助晚期非小细胞肺癌化疗的临床疗效。方法:将68例经病理检查确诊的非小细胞肺癌患者随机分为治疗组和对照组各34例。对照组予以标准GP方案进行化疗,3～4周为1个周期,共进行2个周期评价疗效;治疗组在对照组治疗基础上加用扶正保元汤治疗,共连续治疗2个月后评价疗效。结果:治疗组总有效率61.76%,对照组总有效率35.29%,两组总有效率比较差异有统计学意义(P<0.05)。患者生活质量Karnofsky评分治疗组总有效率为58.82%,对照组总有效率32.35%,两组比较差异有统计学意义(P<0.05)。治疗后外周血T淋巴细胞亚群中CD3+和CD4+较治疗前明显升高,CD8+明显降低,且明显优于对照组,观察组组内治疗前后比较差异均有显著性(P<0.05),治疗后组间比较差异有显著性(P<0.05)。结论:化疗基础上加用合用益气养阴解毒方在控制瘤灶、提高晚期肺癌患者近期疗效方面显著优于单纯化疗,同时细胞免疫功能也有明显提高,推测扶正保元汤可能通过T淋巴细胞功能发挥其免疫杀伤作用。Objective：To observe the clinical effect of ajuvant Fuzhengbaoyuan Tang in chemotherapy of advanced non-small cell lung cancer.Methods：68 patients pathologically diagnosed with non-small cell lung cancer were randomly divided into the treatment group and the control group with 34 cases in each group.The control group was given the standard GP therapy for chemotherapy for two cycles,3~4 weeks a cycle,for evaluation of clinical effect.Based on the therapy in the control group,the treatment group was given additional Fuzhengbaoyuan Tang for 2 months to evaluate the clinical effect.Results：The total effective rate in the treatment group and the control group was 61.76% and 35.29% respectively,and there was significant difference（P0.05）.According to the Karnofsky scoring of quality of life of patients,the total effective rate in the treatment group and the control group was 58.82% and 32.35% respectively,and there was significant difference（P0.05）.After treatment,the percentage of CD3＋ and CD4＋cells among the T lymphocyte subsets from peripheral blood was significantly increased,while the percentage of CD8＋cells was significantly decreased in the treatment group than that in the control group;there was significant difference between that before and after treatment within each group（P0.05） and between the two groups（P0.05）.Conclusion：Chemotherapy combined with ‘Yiqi Yangyin Decoction’ has a better efficacy in controlling tumor spread and improving recent treatment effect in patients with advanced lung cancer than simple chemotherapy.Meanwhile,the cellular immune response was also obviously improved,inferring that Fuzhengbaoyuan Tang executes its immune cytotoxicity by regulating the function of T lymphocytes.
