颌间牵引钉联合微型钛板内固定在颌骨骨折治疗中的应用

摘　　要：目的探讨利用颌间牵引钉结合微型钛板坚固内固定治疗颌骨骨折的临床疗效,注意事项及临床适应症的掌握。方法对2008年8月至2011年10月在我院口腔颌面外科住院治疗的68例颌骨骨折病例应用颌间牵引钉牵引复位结合微型钛板坚固内固定治疗,总结分析临床疗效及操作注意事项,评判其优缺点。结果 68例患者术后创口Ⅰ期愈合折线,其中61例达到解剖复位标准,7例基本复位,咬合关系恢复正常。颌间牵引钉适合于多种类型颌骨骨折,操作时间短,仅局部有轻微损伤,可进行多方向调整,术后患者舒适,拆除方便,但患者经济负担加大。结论颌间牵引钉结合微型钛板坚固内固定治疗颌骨骨折手术快捷、安全、效果良好,为颌骨骨折治疗提供了新的颌间牵引方法,在患者经济条件允许的情况下值得临床推广。Objective %To investigate the use of intermaxillary traction nail combined with rigid internal fixation with mini titanium plate in treatment of mandibular fractures,matters needing attention and clinical indications of the master.Methods From 2008 August to 2011 October in our hospital of oral and maxillofacial surgery in treatment of 68 cases of mandibular fractures in case application of intermaxillary traction nails traction reduction and rigid internal fixation with mini titanium plate in treatment of,summarize and analyze the clinical curative effect and the matters needing attention during operation,evaluation of its advantages and disadvantages.Results Sixty-eight cases of patients with postoperative wound healed broken line,of which 61 cases to achieve anatomic reduction standard,7 cases are reset, occlusal relations back to normal.Intermaxillary traction screw suitable for multiple types of jaw fracture,operation time is short,only local to a slight injury,can make the multiple direction adjustment,postoperative patient comfort,ease of removal,but the economic burden of the patients increased.Conclusion The intermaxillary traction nail combined with rigid internal fixation with mini titanium plate in treatment of mandibular fracture operation fast,safety,effect is good,for the treatment of mandibular fractures provides a new intermaxillary traction method,in a patient with economic conditions allow worthy of clinical application.
