1087例三叉神经痛患者临床资料回顾分析

摘　　要：目的总结1087例三叉神经痛患者的临床资料特点。方法对就诊患者的临床资料进行统计分析,并与之前发表的可查相关文献报道结果做比较。结果 27例患者诊断为症状性(继发性)三叉神经痛。经典性(原发性)三叉神经痛患者以50～70岁年龄段为发病高峰;女性患者多见;罹患侧别以右侧居多;以单侧Ⅱ、Ⅲ支单独受累常见;7例患者存在家族病史;约有19.8%的患者至少合并高血压、糖尿病或冠心病等系统性疾病中的一种。结论就诊患者需行必要的神经系统及影像学检查,以排查症状性三叉神经痛可能。经典性三叉神经痛患者在疼痛特点、发病年龄、性别、罹患侧别、分支等分布规律与之前相关文献报道接近一致。高血压、糖尿病或冠心病等疾患在就诊患者中高发,可能为三叉神经痛发病的诱发因素,但具体有待进一步研究证实。Objective To summarize the clinical features of trigeminal neuralgia in 1087 cases.Methods Demographic data of patients were reviewed,the results of the survey were analysed and comparisons made with former studies.Results 27 cases were diagnosed as symptomatic（secondary）trigeminal neuralgia.Among patients with classic（primary） trigeminal neuralgia,the peak age at onset was between the fifth and seventh decades of life,presented predominantly in females,right-sided involvement occurred at a greater frequency;the maxillary or mandibular division was more frequently involved branch;7 cases had a familial history;19.8% of the patients were affected with hypertension,diabetes and（or） coronary heart disease.Conclusion Neurologic and imaging check were essential to distinguish symptomatic neuralgia.There was a general similarity in the findings as literatures obtained in classic trigeminal neuralgia.Hypertension,diabetes and coronary heart disease maybe a risk factor for TN development,however, further studies were needed to elucidate the underlying mechanism.
