速避凝预防白内障术后眼内炎症反应的临床随机对照研究

摘　　要：目的　研究低分子量肝素 -速避凝对白内障术后眼内炎症反应的预防作用。方法　 110例老年性白内障随机分为 2组 ,并盲法给药。在进行白内障囊外摘除及后房人工晶体植入手术中 ,对照组灌注液为乳酸钠林格氏液 ,实验组灌注液为乳酸钠林格液中加入速避凝浓度为 6U/ml。术后 1、3及 7d观察眼内纤维蛋白渗出、房水混浊及眼内出血情况。结果　实验组术后第 1、3、7d眼内纤维蛋白渗出及房水混浊轻于对照组 (P <0 .0 5 ) ,两组术中及术后出血差异无显著性 (P >0 .1)。结论　速避凝能有效抑制白内障术后眼内炎症反应 ,并且不增加眼内出血危险性。Objective The preventive effects of fraxiparine on intraocular inflammation after extracapsular cataract extraction were studied.Methods One hundred and ten cases with senile cataract were randomly assigned into two groups.The surgery of extracapsular cataract extraction and intraocular lens (IOL) implantation were performed in a masked fashion on control eyes with lactated Ringer's infusion and on expeiment eyes treated with infusate of fraxiparine at a concentration of 6U/ml.The intraoperative bleeding was graded by surgeon.The amounts of fibrin and inflammatory cells and hemorrhage were graded by using slit-lamp examination on the 1 st ,3 rd and 7 th postoperative day.Results The mean grade of fibrin and inflammatory cells in the experiment eyes were lower than that in the control eyes ( P <0.05).No statistically significant differences in the degree of intraoperative or postoperative hemorrhage were observed between the two groups.Conclusions Fraxiparine is an effective inhibitor of postoperative inflammation after extracapsular cataract without inducing ocular bleeding complications.
