胃内巨大结石诊断1例

摘　　要：胃巨大结石在临床上较少见,而胃内结石进入结肠后导致肠梗阻更为少见,临床处理困难较大.现报道1例异常坚硬巨大胃石,男,29岁,因左上腹痛4d入院,胃镜检查示:胃体:见约8-9cm×6-7cm巨大胃石占据整个胃体.腹部立位平片示:中上腹部新增多个长短不一、高低不等的液气平面.结肠内见大量气体影,肠梗阻征象.经胃石切割碎石器,分多次圈套结石表面及结石体部,仅能切割崩解结石表面,结石体部无法完全切割.最终采取胃壁切开异物取出术,术中见胃内有一包块,约10cm×4cm×4cm大小,质硬,灰褐色,将胃内包块自切口取出.术后恢复良好.Large gastric stone is a rare disease,and large gastric stone causing colonic obstruction is even rarer and is very hard to treat.Here we report a case of large gastric stone in a 29-year-old man.He developed left upper abdominal pain four days before admission,and gastroscopy showed that a large gastric stone measuring 8-9 cm × 6-7 cm almost occupied the entire gastric body.Abdominal orthostatic plain X-ray showed multiple air fluid levels in the upper abdomen,immense gas shadow,and sign of intestinal obstruction.Cutting lithotriptor could not completely cut the gastric stone.Finally,the large gastric stone measuring about 10 cm × 4 cm × 4 cm was removed by cutting open the gastric wall.The patient recovered well after surgery.
