摘　　要：目的探讨原发胃肠道的转移性卵巢癌的临床病理特征、治疗方法与预后因素。方法回顾性分析江苏省肿瘤医院妇瘤科从1999年10月～2009年9月收治的45例胃肠道来源的转移性卵巢癌患者的临床病理资料、治疗及预后因素。结果患者中位年龄44岁。原发病灶来源于胃者33例,来源于结直肠者12例。68.9%为绝经前期者,双侧卵巢转移者占64.4%。治疗均采用手术联合化疗的综合方案。全组患者生存时间为3～58月,中位生存15.0月,1、2、3、5年生存率分别为60.0%、15.6%、6.7%和0。单因素分析显示,原发灶部位、转移灶范围、肿瘤细胞减灭术、化疗和卵巢转移癌组织学类型是影响患者预后的主要因素;多因素分析显示,原发灶部位、转移灶范围、肿瘤细胞减灭术和卵巢转移癌组织学类是影响卵巢转移癌患者预后的独立因素。结论原发胃肠道的卵巢转移性癌恶性程度高,预后差。在切除原发肿瘤的同时,行满意的肿瘤细胞减灭术能改善预后。Objective To analyze the clinical pathological features,treatment and prognosis of metastatic ovarian carcinoma from gastrointestinal tract.Methods To review and analyze the clinical pathological features,treatment and prognosis of metastatic ovarian carcinoma from gastrointestinal tract who received operation in our hospital from October 1999 to June 2009.Results The median age of 45 patients was 44 years old.Thirty-three metastatic ovarian carcinoma patients were from stomach,and 12 from colon and rectum.The majority of these patients was in the premenopausal state（68.9%） and had bilateral ovarian involvement（64.4%）.All of them underwent surgical treatment and chemotherapy.The median overall survival time of 45 patients was 15.0 months.Survival rates of 1-,2-,3-and 5-year were 60.0%,15.6%,6.7%,and 0,respectively.Univariable analysis revealed that different origins,extent of recurrent disease,cytoreductive surgery,chemotherapy and pathologic type of metastatic ovarian carcinoma were correlated with the prognosis.In multivariable analysis,different origins,extent of recurrent disease,cytoreductive surgery and pathologic type of metastatic ovarian carcinoma were confirmed to be independent factor.Conclusions Patients with metastatic ovarian carcinomas have poor prognosis.Optimal cytoreduction is associated with improved survival.
