脑卒中患者抑郁情绪与认知功能的相关性研究

摘　　要：目的探讨脑卒中患者抑郁情绪与认知功能的相关性，为临床干预提供依据。方法将50例脑卒中患者设为研究组，抽取48名健康体检者设为对照组，采用汉密顿抑郁量表及抑郁自评量表评定抑郁状况，探究性眼动分析测评认知功能，对测评结果进行相关分析。结果研究组汉密顿抑郁量表及抑郁自评量表评分显著高于对照组（P〈0．01）。研究组探究性眼动分析测评异常率为86％，凝视点数及反应性探索评分显著低于对照组（P〈0．01），D值显著高于对照组（P〈0．01）。研究组汉密顿抑郁量表及抑郁自评量表总分与探究性眼动分析的凝视点数及反应性探索评分呈显著负相关（P〈0．01），与探究性眼动分析的D值呈显著正相关（P〈0．01）。结论脑卒中患者存在一定程度的抑郁情绪及认知功能损害，且抑郁情绪越明显，认知功能障碍越严重。Objective To explore the correlation of depressive and cognitive function in stroke patients in order to provide basis for clinical intervention. Methods Fifty stroke patients were assigned to research group and selected 48 healthy examinees to control group, depression states were assessed with the Hamil- ton Depression Scale （HAMD） and Self-rating Depression Scale （SDS）, cognitive functions with the exploratory eye movement （EEM）, and evaluation results contrastively analyzed. Results The scores of the HAMD and SDS were significantly higher in research than in control group （P〈0.01）. The abnormal rate of EEM was 86% in research group,numbers of eye fixation （NEF）and the responsive search score （RSS） of EEM were significantly lower （P〈0.01） and D value was significantly higher （P〈0. 01） compared with control group （P〈0.01）. The total scores of the HAMD and SDS were notably negatively related to the NEF and RSS （P〈0.01） and positively to D value （P〈0.01）. Conclusion Stroke patients have a certain degree of depressive emotion and cognitive function lesion, and the more obvious depressive emotion is, the severer cognitive function impairment is.
