综合性干预对首发精神分裂症患者生活质量的影响

摘　　要：目的探讨综合性干预对首发精神分裂症患者生活质量的影响。方法将108例首发精神分裂症患者随机分为两组，每组54例，研究组完成51例，对照组完成49例，脱落病例不计人研究结果。两组均常规应用抗精神病药物治疗，研究组在此基础上联合综合性干预，观察1a。于干预前及干预1a末采用阳性与阴性症状量表和生活质量综合评定问卷评定临床疗效。结果干预1a末，研究组阳性与阴性症状量表总分及各因子分和对照组总分、阳性症状及阴性症状因子分均较干预前显著降低（P〈0．05或0．01），且研究组总分、阳性症状及阴性症状因子分均较对照组下降更显著（P〈0．01）；研究组生活质量综合评定问卷总分、躯体功能、心理功能、社会功能维度分和对照组总分、社会功能维度分均较干预前显著升高（P〈0．05或0．01），研究组总分、心理功能及社会功能维度分均较对照组升高更显著（P〈0．05）。结论药物治疗联合综合性干预能有效改善首发精神分裂症患者的精神症状，提高患者的自知力、治疗依从性及生活质量，优于单用药物治疗。Objective To explore the effects of comprehensive intervention on quality of life of first-episode schizophrenics. Methods A total of 108 first-episode schizophrenics were randomly assigned to two groups of 54 ones each, 51 completed in research and 49 did in control group. Both groups received routine antipsychotic treatment, research group was plus comprehensive intervention for 1 year. Efficacies were assessed with the Positive and Negative Syndrome Scale （PANSS） and Generic Quality of Life Inventory-74 （GQOLI-74） at baseline and at the end of 1 year. Results At the end of 1 year total and each factor score of the PANSS in research and total, positive and negative symptom score in control group lowered more significantly compared with pretreatment （P〈0.05 or 0.01）, so did total, positive and negative symptom score in research than in control group （P〈0.01）; total, somatic, psychological and social function score of the GQOLI-74 in research and total and social function score in control group increased more significantly compared with pretreatment （P〈0.05 or 0.01）, so did total, psychological and social function score in research than in control group （P〈0.05）. Conclusion Pharmacotherapy plus whole course comprehensive intervention could effectively improve mental symptoms of first-episode schizophrenics, raise patients＇ insight, treatment compliance and quality of life, and has an advantage over simple pharmacotherapy.
