药物干预对早期正常眼压性青光眼预后的影响

摘　　要：目的探讨药物干预对早期正常眼压性青光眼(normal tension glaucoma,NTG)预后的影响。方法对17例(34眼)早期NTG患者进行前瞻性研究,随机分成2组,一组为对照组8例(16眼)不予药物治疗,另一组为治疗组9例(18眼),给予常规药物治疗。随诊1a,记录患者视力、眼压、眼底及视野情况并进行分析。结果随访1a后,对照组视力较用药前下降(分别为(0.64±0.14和0.53±0.13,P=0.031)、眼压差异无统计学意义[分别为(17.33±1.73)mmHg(1kPa=7.5mmHg)和(18.26±1.05)mmHg(P=0.076)]、杯盘比值上升(分别为0.64±0.09和0.75±0.10,P=0.003)、视野旁中心暗点范围扩大;治疗组视力较治疗前上升(分别为0.71±0.13和0.83±0.15,P=0.014)、眼压下降明显[分别为(17.12±1.79)mmHg和(12.39±1.67)mmHg,P=0.000)、杯盘比值无变化(分别为0.66±0.08和0.67±0.08,P=0.830)、16眼视野旁中心暗点范围缩小或消失。结论 NTG可导致视功能进行性恶化,尽早药物干预可改善患者预后。Obleetive To observe the effects of drug intervention on prognosis of early normal tension glaucoma（NTG）. Methods Seventeen cases（34 eyes） with early NTG were included in this randomized controlled trial. All the eyes were divided into con- trol group（ 16 eyes） and treatment group （18 eyes）. No treatment was given to control group. The treatment group was given conventional drug intervention. The visual acuity, intraocuiar pressure,fundus and visual field were observed. All cases were followed up for 1 year. Results After 1 year follow-up,in control group,the visual acuity was signif- icantly decreased from 0.54 ±0.14 to 0.53 ±0. 13（P =0. 031 ） ,the intraocular pressure was maintained in normal ranges from（ 17.33 ±0 1.73） mmHg（ 1 kPa =7.5 mmHg） and（ 18.25 ±0 1.05 ） mmHg （P = 0.075）, the ratio of cup to disc was increased from 0.54 ±0.09 to 0.75 ±0.10（P =0. 003） ,and the ranges of paracentral scotoma in the visual field defect were enlarged. In treatment group ,the visual acuity was significantly increased from 0.71 ±0. 13 to 0. 83 ±0. 15 （P = 0. 014）, the intraocular pressure was significantly decreased from （ 17. 12 ± 1.79） nmHg to（ 12.39 ±1.57） mmHg（P = 0. 000） ,the ratio of cup to disc had nochange from 0.56 ±0 0.08 to 0.67 ±0 0.08 （P = 0. 830）, and the ranges of paracentral scotoma in the visual field defect were less- ened or disappeared. Conclusion The visual function of NTG will deteriorate progressively without intervention, and the prognosis can be improved if the drug intervention is conducted as soon as possible in early NTG.
