摘　　要：目的比较常规苏木素-伊红(HE)染色、连续切片(SSS)-HE染色和SSS-免疫组织化学(IHC)3种病理学检查方法对兔头颈部肿瘤颈部淋巴结隐匿性转移的检测能力,分析其敏感性。方法兔头颈部肿瘤淋巴结转移模型建立3周后,分离20只实验动物颈部所有淋巴结,进行HE染色和IHC检测。分析不同病理检查方法检测淋巴结隐匿性转移存在与否,转移的数量和类型。结果从20只实验兔中分离出颈部淋巴结128个,隐匿性淋巴转移8个,常规HE染色检测出4个(3.1%),SSS-HE染色检测出6个(4.7%),SSS-IHC8个(6.3%)隐匿性淋巴转移均检出,3组间隐匿性淋巴结的检测率差异有统计学意义(P<0.05)。3种方法均未检出单个的肿瘤细胞。结论常规HE染色切片仅能检出体积较大的隐匿性淋巴结转移。SSS-HE染色能检出体积较小的微转移、部分孤立肿瘤细胞。SSS-IHC能检出包括全部孤立肿瘤细胞的隐匿性淋巴结转移,是研究隐匿性淋巴结转移重要方法。Objective To compare the diagnostic capacity of conventional haematoxylin-eosin （HE） staining, serial step sectioning （SSS）-HE staining and SSS-immunohistochemistry （IHC） on occult metastasis of cervical lymph node in rabbits with head and neck tumor for determination of the assay sensitivity. Methods Following a 3-week period for establishment of animal models （n=20）, the lymph nodes were disected for fixation in formalin and pro- cessed for HE staining and IHC. Various pathological assays were employed for determination of the presence of lymph node occult metastasis, the number of metastasized nodes and type of metastasis. Results Of 128 lymph nodes isolated from 20 rabbits, 8 had occult metastasis, of which 4 （3.1%） were confirmed by routine HE staining, 6 （4.7%） were proved by SSS-HE staining and 8 （6.3%） were diagnosed by SSS-IHC （P〈0.05）. The single tumor cell was de- tected by none of the measures. Conclusion Whilst massive occult lymph node metastasis could be assayed by rou- tine HE staining, SSS-HE staining may detect limited micro-metastasis or orphan tumor cells. SSS-ICH is capable of detecting all lesions, including orphan tumor ceils, and is a crucial approach for studying occult lymph node metasta- sis.
