摘　　要：【目的】探讨HPV16／18和survivin在宫颈癌进展中的相关性及临床意义。【方法】选择高危宫颈癌患者108例做常规宫颈脱落细胞和分泌物PCR-HPV-DNA检查，并在阴道镜下行宫颈多点活检做病检和免疫组化，比较不同分级HPV感染阳性率和survivin蛋白表达阳性率，分析HPV-DNA基因表达与sur-vivin蛋白表达的相关性。【结果】宫颈上皮内瘤样病变Ⅰ级（CINⅠ）、CINⅡ、CINⅢ级和宫颈癌患者的宫颈HPV16／18感染阳性率分别为42．42％、61．90％、68．42％和82．86％，显著高于正常体检者的15．00％，其差异有统计学意义（P＜0．05）；CINⅠ、CINⅡ、CINⅢ级和宫颈癌患者的宫颈survivin蛋白表达阳性率分别为81．82％、90．48％、100％和100％，显著高于正常体检者阳性率的10．0。％，其差异均有统计学意义（P〈0．05）；CINⅠ、CINⅡ、CINⅢ级和宫颈癌患者的HPVl6／18-DNA基因表达与survivin蛋白表达存在正相关（P〈0．05）。【结论】HPVl6／18和survivin与宫颈癌进展关系密切，且均随CIN分级和宫颈癌级别的升高而表达增加，两者联合检测有利于提高宫颈癌的诊断率。[Objective] To explore the relationship between HPV16/18 and survivin in the progression of cervical cancer and the significance. [Methods] Conventional cervical exfoliated cells and secretions PCR- HPV-DNA of 108 patients with high-risk cervical cancer were examined. Multiple biopsies of cervix were per- formed under colposcope for pathologic examination and immunohistochemistry. The positive rate of HRV and survivin were compared among different grades. The correlation between HPV-DNA and survivin expression was analyzed. [Results]The positive rate of HPV16/18 in cervix of patients with cervical intraepithelial neo- plasia（CIN） Ⅰ , CIN Ⅱ, CINⅢ and cervical cancer were 2. 420%, 61.90%, 68.42%and 82.86% respectively, which were markedly higher than those in normal healthy controls（15.00 %）, and there were significant differ- ences（ P 〈0.05）. The positive rate of survivin protein in cervix of patients with CIN Ⅰ , CIN Ⅱ , CINⅢ and cervical cancer were 81.82%, 90.48%, 100%and 100% respectively, which were markedly higher than those in normal healthy controls（10. 00%）, and there were significant differences（ P 〈0.05）. The expression of HPV16/18-DNA in patients with CIN Ⅰ , CIN Ⅱ , CINⅢ and cervical cancer had positive correlation with the expression of survivin（ P 〈0.05）. [Conclusion] HPV16/18 and survivin are closely related to the progression of cervical cancer, and increase with the increasing of the grade of CIN and cervical cancer. The combinationdetection of HPV16/18 and survivin are helpful to improve the diagnosis rate of cervical cancer.
