摘 要:目的:通过食管癌术后病人应用肠内免疫营养制剂观察营养状况和免疫功能的影响。方法:将60例食管癌病人随机分为试验组(n=30)和对照组(n=30)。试验组于术后第2天开始给予肠内免疫营养制剂(瑞能)至术后第7天;对照组术后给予全肠外营养。两组病人分别于术前1 d、术后第2和第7天分别检测血清清蛋白(ALB)、前清蛋白(PA)、免疫球蛋白(IgG、IgA、IgM)、T淋巴细胞亚群(CD3+、CD4+、CD8+)水平。同时观察两组病人胃肠功能恢复情况、肝功能和术后感染性并发症的差异。结果:试验组病人术后胃肠功能和肝功能恢复得更早,并发症的发生率明显低于对照组(P<0.05)。试验组病人PA、IgG、IgA、CD3+、CD4+等营养和免疫指标明显升高,与对照组比有显著性差异(P<0.05)。结论:食管癌病人术后应用肠内免疫营养制剂能促进胃肠道功能尽早恢复,改善病人术后的营养状况和免疫功能。Objective: To observe the effect of enteral immune nutrient(supportan) on nutriture and immune function in postoperative patients with esophageal cancer.Methods:Sixty patients with esoghagus cancer were randomly divided into treatment group(n=30)and control group(n=30).Patients of the treatment group were given the enteral nutrient(supportan) from the postoperative 2nd day to the 7th day,while the control group with TPN.Patients of the two groups were checked for levels of ALB,PA,IgG,IgA,IgM,T lymphocyte subsets(CD3+,CD4+,CD8+) respectively on the preoperative 1st day,the postoperative 2nd and 7th days.The recovery of gastrointestinal function,liver function,and the postoperative infectious complication were observed.Result:The recovery of gastrointestinal and liver function in the treatment group was earlier than the control group.The incidence of complication was obviously lower in the treatment group than in the control group.There were significant differences in PA,IgG,IgA,CD3+,CD4+ between two groups(P0.05).Conclusion: The enteral immune nutrition can promote gastrointestinal function recovery and improve the nutrition and immune function in the postoperative patients with esophayeal cancer.
