摘 要:目的:探讨含缓释淀粉的肠内营养(EN)乳剂在高龄非糖尿病食管癌病人术后的应用价值。方法:选择高龄非糖尿病食管癌手术病人126例,随机分为缓释淀粉EN乳剂组(研究组,n=68)和标准EN乳剂组(对照组,n=58)。两组病人术后EN均采用输注泵输注,高血糖者给予外源性胰岛素控制血糖。观察两组病人血糖变化情况、胰岛素用量以及术后并发症。结果:研究组病人的血糖波动值小和胰岛素用量少,院内感染发生率低,与对照组比较有显著性差异(P<0.05)。结论:含缓释淀粉的EN乳剂较标准EN乳剂更适合高龄非糖尿病食管癌病人术后的营养支持。Objective: To evaluate the clinical application of enteral nutrition(EN) emulsion with slow-release starch in elderly nondiabetic patients with esophageal cancer aged 70 years and over.Methods:126 elderly nondiabetic patients after esophagectomy were randomly divided into study group(EN emulsion with low-release starch,68 cases) and control group(standard EN emulsion,58 cases).The EN infusion pump was used in both groups.Blood glucose was controlled with continuing insulin infusion.The patient's blood glucose level,total insulin dose within 5 days of EN and postoperative complications were observed.Results: Compared with control group,the fluctuations of blood glucose,total insulin dose within 5 days of EN and the incidence of infection in hospital in the study group were significantly lowered.Conclusion: EN emulsion with slow-release starch is more suitable for elderly nondiabetic patients with esophageal cancer aged 70 years and over postoperatively.
