摘  要:目的分析上海市嘉定区胃癌发病的危险因素,为该区胃癌的干预提供科学依据。方法选取2010年5月至2011年8月上海市嘉定区诊断新发胃癌病例224例。进行1:1全人群配对病例对照研究。结果单因素分析结果显示,重盐饮食,腌制食品,常吃宵夜,调料糖、料酒、味精使用较多,吸烟,饮酒,经常失眠,体力劳动强度过大,胃部疾病家族史,肿瘤家族史可增加患胃癌的风险;而多吃海产品,调料醋、辣椒、芡粉使用较多,新鲜蔬菜,豆制品,饮茶,经常散步,性格开朗则是胃癌的保护性因素。多因素分析结果发现,吸烟、常吃腌制食品、胃息肉患病史和家族肿瘤史是影响该区居民胃癌发生的主要危险因素,而常饮茶、多吃新鲜蔬菜、多吃豆类食品、性格开朗对于胃癌的发生有保护性作用。结论不当的饮食习惯和生活方式是该区居民胃癌高发的危险因素,积极在全人群开展健康教育,普及防癌抗癌知识,倡导健康的饮食习惯和生活方式对控制该区胃癌发生发展有重要的意义。Objective To study the major risk and protective factors for stomach cancer in Jiading district, Shanghai. Methods Population-based case-control study was conducted in Jiading, 224 new cases of stomach cancer from May 2010 to August 2011 were matched in 1:1 with normal population controls. The data were statistically analyzed by fitting-conditional logistic analysis. Results The results of single factor analysis indicated that heavy salt diet, pickled food, frequently midnight snack, using too much of sugar, cooking wine and monosodium glutamate, smoking, drinking, excessive intensity of physical labour, family history of the stomach disease, family history of cancer could increase the risk of stomach cancer. In addition, high intakes of seafood, vinegar, pepper, cooking starch, fresh vegetables, beans and tea, as well as more walk and outgoing personality were the protective factors for the stomach cancer. Results from multiple conditional logistic regression analysis showed that smoking, often intake of pickled food, history of stomach polyp and family history of cancer were risk factors for stomach cancer, but, frequent intake of fresh vegetables, bean products, tea and optimistic character could decrease the risk for stomach cancer. Conclusion Improper diet and lifestyle are the risk factors for stomach cancer in residents of this area. It is beneficial for decreasing the incidence of stomach cancer to strength the health education in high stomach cancer incidence areas, to promote the health habit of diet and behavior into people' s daily life.
