绿激光同期治疗前列腺增生合并浅表性膀胱肿瘤的临床疗效观察

摘　　要：目的评价分析绿激光同期治疗前列腺增生合并浅表性膀胱肿瘤的临床疗效。方法回顾性分析我院同期行经尿道绿激光前列腺汽化术加浅表性膀胱肿瘤汽化术的16例患者的临床资料。结果所有患者均手术成功，术中出血少，无一例发生膀胱穿孔、闭孔神经反射及输尿管开口损伤，术后平均住院时间为（4．7±1．0）d。术后1个月国际前列腺症状评分由术前（30．0±3．0）分下降至（12．0±2．0）分，最大尿流率由（5．0±2．0）ml／s上升至（17．5±3．5）ml／s，生活质量评分由（5±1）分下降至（2±1）分。术后未出现尿道狭窄和膀胱癌前列腺窝内种植转移。1例患者于术后6个月膀胱肿瘤复发，再次行经尿道绿激光手术，随访24个月未复发。结论经尿道绿激光同期治疗前列腺增生合并浅表性膀胱肿瘤，出血少，无前列腺窝种植转移，无严重并发症，疗效确切，同时可提高患者生活质量。Objective To evaluate the clinical efficacy of green laser treating benign prostate hyperplasia with superficial bladder tumor. Methods The data of 16 cases with benign prostate hyperplasia combined with superficial bladder tumor treated by green laser were retrospectively analyzed. Results All patients undertook successful surgery. No bladder perforation, obturator nerve reflex and ureter injury occurred. Postoperative hospital stay was （4.7 ± 1.0） days. One month after operation, the international prostate symptom score （IPSS） decreased from（30.0 ±3.0） scores to（ 12.0 ± 2.0） scores. Maximum flow rate increased from the（5.0 ± 2. 0） ml/s to （ 17.5 ± 3.5 ） ml/s. Quality of life score decreased from the （ 5 ± 1 ） scores to （ 2 ± 1 ） scores. The urethral stricture did not appear and the prostate fossa metastasis from bladder tumor did not happen. Six months after treatment, one patient had bladder tumor recurrence and had re-green laser transurethral surgery, and recovered. Conclusions The transurethral green laser treating benign prostatic hyperplasia with superficial bladder tumors is safe and effective, meanwhile it can improve qulity of life.
