顺铂腹腔热灌注联合紫杉醇静脉输注治疗进展期卵巢癌的观察及护理

摘　　要：目的总结顺铂腹腔热灌注联合紫杉醇静脉输注并配合腹部热疗治疗进展期卵巢癌的临床资料。方法回顾性分析2007年3月至2009年2月在浙江省肿瘤医院18病区治疗的42例进展期卵巢癌患者的临床资料,所有患者均采用紫杉醇175mg/m2静脉输注,顺铂75mg/m2腹腔热灌注,腹部射频热疗为43℃、60min、2次/周,21d为1个疗程。整个治疗过程中加强心理护理,重视综合治疗过程中的多环节护理。结果本组患者治疗有效率为64.3%,生活质量改善率为90%,肿瘤指标CA125的水平,治疗前为(376.05±54.73)U/ml,治疗后为(173.73±52.21)U/ml。结论进展期卵巢癌患者在顺铂腹腔热灌注联合紫杉醇静脉输注并配合腹部热疗方案的治疗过程中应加强心理护理、密切观察、及时处理不良反应,有利于提高疗效。Objective To sum up the nursing experience of hyperthemic intraperitoneal perfusion theraphy of cisplatin combined with intravenous infusion of paclitaxel in the treatment of advanced ovarian cancer. Methods From March 2007 to February 2009, the nursing countermeasures were analyzed retrospectively in 42 advanced ovarian cancer patients treated （60 rain at a time, 2 times a week, 21 d） of paclitaxel intravenous chemotherapy （175mg/m2 ）, hyperthemic intraperitoneal perfusion theraphy of cisplatin （75 mg/m2）and abdominal thermotherapy（43~C）. The psychological care was strengthened with the focus to every link in the whole process. Results The total efficiency was 64.3~/00 . The improvement rate of their quality of life was 90 ~. The levels of CA125 before and after the treatment were（173.73± 52.21）U/ml and （376. 05 ± 54. 73） U/ml, respectively. Conclusion Strengthening the psychological care, attaching more attention to every link in the whole process and timely treating the adverse reactions can effectively improve the therapeutic effect of advanced ovarian cancer patients undergoing intravenous infusion of pacli- taxel and hyperthemic intraperitoneal perfusion chemotheraphy of cisplatin combined with abdominal ther- motherapy.
