摘　　要：目的　观察透明质酸钠 (SH)预防使用丝裂霉素C的首次小梁切除术后早期低眼压综合征 (低眼压、浅前房、低眼压性黄斑病变、脉络膜脱离等 )的效果。方法　首次接受小梁切除术的 74眼 ,术中均一次性使用 0 3mg/ml的丝裂霉素C ,其中 3 6眼在对角缝合巩膜瓣 2针后于瓣下及前房内注入SH 0 2ml ,另外 3 8眼则注入B .S .S .溶液进行对照。结果　术后早期SH组有 4眼出现浅前房 ,1眼发现脉络膜脱离 ;对照组则有 19眼出现浅前房 ,7眼发现脉络膜脱离 ,同时有 4眼出现低眼压性黄斑病变 ,而且其术后早期低眼压也较SH组明显。结论　术中巩膜瓣下及前房内注入少量SH ,能有效预防使用丝裂霉素C的首次小梁切除术后早期低眼压综合征的发生。ObjectiveTo observe the effects of sodium hyaluranate(SH)preventing early hypotony syndrome(hypotony,flat anterior chamber,hypotony maculopathy and choroid detachment,etc.)after mitomycin C(MMC) trabeculectomy as primary surgery.Methods 74 eyes were treated with MMC trabeculectomy,SH(0.2?ml)was injected into the anterior chamber and under the sutured scleral flaps in 36 eyes,respectively;the other 38 eyes were injected with B.S.S.solution.ResultsAfter operation,there were 5 eyes with flat anterior chamber and 1 eye with choroid detachment in SH group,and the corresponding number of eyes were 19 eyes and 7 eyes in control group. In addition,4 eyes with hypotony maculopathy were found in control group.And compared with SH group,the early postoperative intraocular pressure of eyes in control group were reduced more remarkedly.ConclusionsA small amount of SH injected into anterior chamber and under sutured scleral flap can prevent the eyes from early postoperative hypotony syndrome in MMC trabeculectomy as an initial surgery.
