摘 要:目的检索和分析国内外乳腺癌的MRI应用研究文献,了解MRI技术在乳腺癌诊断和治疗中的应用及发展情况,为其临床实践与循证研究提供参考依据。资料与方法在CBMdisc、PubMed、Cochrane图书馆和NGC指南数据库中检索所有乳腺癌的MRI应用研究方面的文献,按照发表年限、证据级别、MRI技术、临床应用等特征对文献进行统计分析。结果①乳腺癌的MRI应用研究文献呈逐年稳步增长的趋势,国外增速高于国内。国内外关于乳腺癌的MRI研究文献量占乳腺癌的影像学研究文献量的百分比增长节奏和趋势大致一致。②国外发表的关于乳腺癌的MRI应用研究的高证据级别文献明显多于国内,起步早于国内。③动态增强和灌注成像技术在国内外乳腺癌诊治中均占明显优势,其次国内侧重扩散加权成像技术,国外则侧重磁共振波谱技术。国内MRI技术的联合使用及MRI与其他影像技术联合使用的研究文献百分比均高于国外。④国内外MRI均主要应用于乳腺癌的诊断,其次为乳腺癌的治疗,再次为检测转移和复发。结论①MRI技术已成为乳腺癌诊断的新技术,多种影像技术综合应用的乳腺癌影像诊断系统正在形成。②MRI技术以动态增强和灌注成像为主,多种MRI功能成像技术联合使用更具有诊断价值。③MRI主要应用于乳腺癌诊断,评估新辅助化疗疗效是研究热点,MRI引导下的乳腺癌穿刺活检和乳腺癌局部切除是研究方向。④国外乳腺癌的MRI检查作为常规检查方法,其规范化、标准化研究尚需加强。我国需要尽快开展乳腺癌的MRI检查的循证研究,提高随机对照试验的数量,并尽快编写适合我国国情的规范化、标准化的实践指南。Purpose To investigate both domestic and international researches on breast MRI, and to reveal the current status of its research and clinical applications in order to provide information for evidence-based research and clinical practice. Materials and Methods All publications about breast MRI were collected from CBMdisc, PubMed, the Cochrane Library and NGC. Statistical analysis was performed according to the publication year, basic or clinical study, publication types and subjects. Results (~) There was a steady increasing of breast MRI publications domestically and abroad while the publications concerning breast MRI cancer showed steady increase year by year. The annual growth rate was lower in China than abroad. The percentage of breast MRI to the breast cancer was consistent between domestic and abroad. (~) There was less and later high grade evidence literature on breast MRI in China than abroad.(~)The dynamic contrast enhancement and perfusion studies were prevalent both in China and abroad. In China, the diffusion weighted imaging was emphasized, while magnetic resonance spectroscopy was emphasized abroad. The ratio of multi-spectral studies was higher in China than abroad. @ Both domestic and international researches focused on diagnosis, the others were therapy evaluation, metastasis and recurrence detection. Conclusion @ MRI is the cutting- edge technology for breast cancer detection, using its multi-spectral capacity. @ The dynamic contrast enhancement and perfusion techniques are major application on breast, and the efficacy can be improved with multi-spectral techniques. (3) The major application of MR1 in breast is diagnosis, the neoadjuvant therapy MRI evaluation is current locus and the MRI guided biopsy and lumpectomy are the research future. @ The standardization of breast MRI should be improved domestically and abroad. In China, the evidence-based breast MR1 is still under investigated. The RCT should be improved and a practice guideline is essential.
