Barrett食管CK7/20的表达方式及其意义

摘　　要：目的研究CK7和CK20在Barrett食管和贲门肠化黏膜表达的差异。方法对19例长节段Barrett食管,36例短节段Barrett食管,20例贲门肠化胃镜标本进行CK7、CK20免疫组织化学染色,分析其与临床病理资料的关系。结果 100%(19/19)长节段Barrett食管、85%(31/36)短节段Barrett食管和10%(2/20)贲门肠化表达Barrett's CK7/20模式。长节段Barrett食管Barrett's CK7/20模式表达方式及病理资料与短节段Barrett食管相似,与贲门肠化明显不同。结论 Barrett食管CK7/CK20表达模式是诊断Barrett食管的客观标志。Objective To compare the expression patterns of cytokeratin 7 （CK7） and CK20 in the Barrett＇s esophagus and gastric cardia intestinal metaplasia. Methods Endoscopic biopsy specimens from 19 patients with long segment Barrett＇s esophagus, 36 with short segment Barrett＇s esophagus, and 20 with histological evidence of gastric cardia intestinal metaplasia were immunostained for CK7 and CK20. The immunohistochemical data were analyzed in relation with the clinicopathological data of the patients including age, hiatal hernia, and Helicobacter pylori status. Results The Barrett＇s pattern of CK7/20 expressions was found in all the 19 patients with long segment Barrett＇s esophagus, in 31 of the 36 （82%） patients with short segment Barrett＇s esophagus, and in 2 of the 20 （10%） patients with gastric cardia intestinal metaplasia. Patients with short segment Barrett＇s esophagus who had a Barrett＇s CK7/20 pattern showed similar clinicopathogical findings to those with long segment Barrett＇s esophagus, while in cases of short segment Barrett＇s esophagus where no Barrett＇s CK7/20 pattern was found, the clinicopathogical features were similar to those of gastric cardia intestinal metaplasia cases. Conclusions A Barrett＇s CK7/20 expression pattern is an objective marker of Barrett＇s mucosa. CK7 and CK20 reactivity patterns in routine endoscopic biopsy samples can reliably identify the location of intestinal metaplasia in the esophagus and stomach.
